超声引导下腹腔肿大淋巴结穿刺术

目的：探讨超声引导穿刺活检对腹腔肿大淋巴结的诊断价值。方法：常规超声检查时发现腹腔内复发或多发原因不明的肿大淋巴结的患者71例。为明确诊断，全部患者行超声引导下腹腔肿大淋巴结穿刺活检术，包括针吸细胞学检查UGAB和组织切割活检UGCB。结果：71例患者以最后确诊结果分为良、恶性两组。其中良性组25例， 恶性组46例。穿刺后证实UGAB诊断率为77．5％，其中良性诊断率68．0％，恶性诊断率为82．2％，良性诊断率与恶性诊断率之间无明显差异P＞0．05：UGCB诊断率为87．3％，其中良性诊断率为76．0％，恶性诊断率为93．3％，良恶性诊断率之间无明显差异P＞0．05。两种方法对所有病例的综合诊断率之间亦无差异P＞0．05。而将两种方法互相结合，互相补充，诊断率明显提高，达95．5％。结论：超声引导下穿刺活检术安全、准确，在腹腔肿大淋巴结的疾病诊断中非常有价值，尤其结合应用UGAB和UGCG两种方法，进一步提高了诊断率。

Value of Ultrasound-guided Needle Biopsy of Abdominal Lymphadenopathy

Objective:To assess the value of ultrasound guided needle biopsy(UGNB)of abdominal lymphadenopathy.Methods:Seventy one Patients with abdominal lymphadenopathy underwent ultrasound guided needle biopsy,including ultrasound guided aspiration biopsy (UGAB)and ultrasound guided cutting biopsy(UGCB)in all the patients.Results:Seventy one patients were divided into two group:benign( n =25)and maligant( n =46).Using UGAB alone,the diagnostic rate was 77 5%,and UGCB alone,the rate was 87 3%.The difference between two methods was not significant( P >0 05).And neither UGAB nor UGCB had significant difference in diagnostic rates between benign group and malignat group(68 0% VS 82 2%, P >0 05;76 0% VS 87 3%, P >0 05).If both UGAB and UGCB were used,the diagnostic rate was 95 5%.Conclusions:We conclude that UGNB is a useful and valuable diagnostic tool,not only safe but also accurate,especially by using the two methods(UGAB,UGCB) together.
