老年人拔牙中麻醉剂复方阿替卡因的合理应用

目的 探讨老年人拔牙中麻醉剂复方阿替卡因（碧兰麻）不同剂量对患者的影响。方法 将480例老年病人随机分为2组，在使用麻醉刑后拔牙并利用VAS（visud analogue scale）标尺法对患者疼痛程度进行打分；口腔科医生评价麻醉效果，统计麻醉完全、良好、有效和无效的比例；同时观察2组接受拔牙病人在麻醉剂使用前和使用10min后的平均动脉压（MBP）和心率（HR），以及不良反应的发生率。结果 2组病人的VAS评分无显著性差异（P〉0．05）；2组病人使用麻醉疗效也无显著性差异（P〉0．05）；但2组病人使用麻醉剂后的MBP和HR有显著性差异（P〈0．05）；2组病人均无明显的不良反应。结论 复方阿替卡因在老年人拔牙中使用是安全有效的，但须减少剂量。

Use of local anesthesia articaine co. in tooth extraction in elderly patients

Objective To evaluate the effect of different doses of primacaine on the elderly patients undergoing tooth extraction. Methods 480 elderly patients were randomly divided into two groups.The visud analogue scale(VAS) was used to scale the pain intensity. The effect of local anesthesia was evaluated by the dentists. MBP and HR were observed before and after drugs administration,and the side effects were observed during the operation. Results There were no significant difference in VAS score and efficacy of local anethesia between two groups. But significant differences of HR and MBP were observed during the whole process. There were no side effects observed. Conclusions Dose of primacaine should be decreased in the elderly patients.It would be an efficient and safe dental anesthetia.
