甲状腺腺瘤多层螺旋CT诊断与临床病理分析

摘    要:目的:对甲状腺腺瘤的多层螺旋CT诊断与临床病理进行分析。方法对21例甲状腺腺瘤患者的多层螺旋CT检查结果进行分析,并对其病理标本进行研究。结果甲状腺结节数为1枚的患者有18例(85.71%),2枚有3例(14.29%);均匀强化有6例(28.57%),不均匀强化有2例(9.52%);出现包膜有13例(61.90%);钙化现象出现2例(9.52%);滤泡状腺瘤15例,乳头状腺瘤6例。发生于甲状腺左叶者10例,甲状腺右叶者5例;跨甲状腺右叶及峡部者2例,跨甲状腺左叶及峡部者1例,跨甲状腺左右叶者3例。病变最大者约4.0 cm ×5.0 cm,检出最小者约1.0 cm ×1.0 cm。病变边缘不清晰者1例,界限清晰者20例。5例正常甲状腺组织受压体积增大。囊性结节4例,占19.05%;实性结节8例,占38.10%;混合性结节9例,占42.86%。结论综合各种CT征象,结合详实可靠的临床表现,有利于甲状腺腺瘤的诊断与治疗。

Abstract:Objective To investigate the value of multi-slice spiral CT for the diagnosis of thyroid adenoma, and to analyze the clini-cal pathology of thyroid adenoma, in order to improve the accuracy of multi-slice spiral CT in the diagnosis of thyroid adenoma. Methods The results of 21 patients with thyroid adenoma by multi-slice spiral CT examination were analyzed, and research on its pathological specimen and slice features was carried out. Results There were 18 cases with single thyroid nodule(85. 71%), and 3 cases with two thyroid nodules (14. 29%). Homogeneous enhancement occurred in 6 cases (28. 57%), and inhomogeneous enhancement in 2 cases (9. 52%). The emer-gence of coated was in 13 cases (61. 90%). Calcification occurred in 2 patients (9. 52%). There were 15 cases of follicular adenoma and 6 cases of papillary adenoma. 10 cases were in the left lobe of the thyroid gland and 5 cases of right lobe. 2 cases were in the inter lobe and isthmus of right thyroid, 1 case across the left thyroid lobe and isthmus of thyroid, and 3 cases cross left lobe. The largest lesions were about 4. 0 cm × 5. 0 cm, and the detectable minimum was 1. 0 cm × 1. 0 cm. The edge is not clear in 1 case of lesions and clear in 20 cases of boundaries. Tissue compression volume increased in 5 cases of normal thyroid . There were 4 cases of cystic nodules, which accounted for 19. 05%;8 cases of solid nodules, accounted for 38. 10%;9 cases of mixed nodules, accounted for 42. 86%. Conclusion The compre-hensive CT findings with clinical manifestations of detailed and reliable analysis, combined with reasonable clinical manifestation, can have beneficial effects on the early discovery and timely diagnosis and treatment of thyroid adenoma.
