瑞芬太尼与芬太尼在腹腔镜直肠癌切除术中的麻醉效果比较

目的比较瑞芬太尼与芬太尼在腹腔镜直肠癌切除术中的麻醉效果。方法选择2005年1月至2009年12月治疗的120例直肠癌患者,随机分为观察组和对照组各60例,观察组为瑞芬太尼麻醉组,对照组为芬太尼麻醉组,比较两组麻醉效果。结果两组患者插管前及插管后5min,HR和MAP无显著变化,插管后20min和插管后1h,观察组HR、MAP均低于对照组(P<0.05),并且较为平稳;观察组自主呼吸恢复时间、苏醒时间、清醒时间及停药至气管拔管时间均短于对照组,两组比较差异有统计学意义(P<0.05)。结论瑞芬太尼对心率、血压等血流动力学有良好的可控性,苏醒迅速安全,用于腹腔镜直肠癌切除是理想的选择。

Anesthetic Effect Compare of Remifentanil and Fentanyl in Rectal Cancer Operating by Laparoscopy

Objective To compare the anesthetic effect of remifentanil and fentanyl in rectal cancer operating by laparoscopy.Method Select 120 rectal cancer patients that treated in A hospital from Jan 2005 to Dec 2009 and randomly divided into observation group and control group,60 patients each group,the observation group are anesthetized by emifentanil while the control group are anesthetized by fentanyl.Results HR and MAP of the two groups are not different before intubate and 5 min after intubate,but after 20 min and 1h,HR and MAP of the observation group is lower than the control group（P0.05）;respiratory recovery time,analepsia time,awake time and extubation time of the observation group is shorter than the control group,the difference has statistical significance（P0.05）.Conclusion Using remifentanil can well control heart rate and blood pressure,patients awake quickly,it is a good choice for rectal cancer operation by laparoscopy.
