臂丛神经肿瘤性病变MRI诊断

目的:了解臂丛神经肿瘤性病变的MRI表现特征。方法:回顾性分析20例臂丛神经肿瘤性病变的MRI表现。结果:原发性肿瘤14例(神经鞘瘤11例,神经纤维瘤3例),肿瘤位于椎间孔、斜角肌间隙和腋窝等部位,T1WI呈略低信号～等信号,T2WI呈较高信号～高信号,不均匀性明显强化。邻近肿瘤侵犯5例,纤维瘤、弹力纤维瘤和白血病各1例,前二者位于斜角肌间隙至腋窝区域,T1WI呈等信号,T2WI呈高信号;后者侵犯臂丛神经及相邻椎体,臂丛神经病变类似三角形,由椎体侧向外下延伸;肺上沟癌2例。乳腺癌转移1例,MRI表现类似白血病。结论:MRI可清楚地显示臂丛神经肿瘤性病变的部位和范围,对大多数肿瘤可以定性诊断。

MR imaging of the neoplastic diseases on the brachial plexus

Objective:To study the MRI characteristics of neoplastic diseases in brachial plexus.Methods:MRI of 20cases of the neoplastic diseases in the brachial plexus were retrospectively analysed.Results:There were14cases of primary tumors (11neurinomas and3neurofibromas)locating at intervertebral foramens,scalene space,and axilla,which mainly showed intermediate signal on T 1 WI and slightly high to high signal on T 2 WI.Five cases of direct invasion by neighboring tumors oc-curred in fibroma,elastofibroma,leukemia and pancoast carcinoma ,with the former two locating at scalene space and axilla,showing intermediate signal on T 1 WI and high signal on T 2 WI.Metastasis was found in1case,similar to leukemia,invaded brachial plexus and neighboring vertebral bodies,with trianglular masses developing from the vertebral side to the brachial plexus.Conclusion:The location and extension of the neoplastic diseases in the brachial plexus could be clearly demonstrated on MRI,and the diagnosis could be correctly made for most tumors according to the characteristics of their invasion and sig-nal intensities.
