急性心肌梗死病人细胞黏附分子与氧化低密度脂蛋白的变化

摘    要:目的探讨急性心肌梗死(AMI)病人细胞黏附分子与氧化低密度脂蛋白(OX-LDL)的变化。方法采用ELISA法检测了69例AMI病人及45例陈旧性心肌梗死(OMI)病人血清OX-LDL、血管细胞间黏附分子-1(sVCAM-1)、细胞间黏附分子-1(sICAM-1)的变化,并与39例健康人进行比较。结果AMI组和OMI组OX-LDL、sICAM-1、sVCAM-1水平明显增高,与对照组比较差异有显著性(F=100.15~3981.19,q=6.82~51.87,P<0.01),且AMI组OX-LDL、sICAM-1、sVCAM-1水平明显高于OMI组,差异有显著性(q=13.20~31.07,P<0.01)。发病第4天死亡者和伴有室性心律失常者的OX-LDL、sICAM-1、sVCAM-1水平明显高于存活者和无室性心律失常者(t=3.39~24.20,P<0.01)。AMI病人OX-LDL与sICAM-1、sVCAM-1水平呈正相关(r=0.729、0.840,P<0.01)。结论细胞黏附分子和OX-LDL参与了AMI的发生,且与病情严重程度和预后有一定关系。

Abstract:ObjectiveTo explore the changes of cell adhesion molecules and oxidative low density lipoprotein(OX-LDL) in patients with acute myocardial infarction(AMI).MethodsBy using ELISA method,the soluble intercellular adhesion molecule-1(sICAM-1),and soluble vascular cellular adhesion molecule-1(sVCAM-1) were measured in 69 AMI,45 OMI patients and 39 controls.ResultsThe concentration of OX-LDL,sICAM-1,and sVCAM-1 in patients with AMI and OMI was higher than that in the controls(F=100.15-3 981.19,q=6.82-51.87,P<0.01),and even higher in AMI patients than that in OMI patients (q=13.20-31.07,P<0.01).The OX-LDL,sICAM-1,sVCAM-1 concentrations were much higher in patients with ventricular arrhythmia and the dead than that in patients without ventricular arrhythmia and survivals(t=3.39-24.20,P<0.01).The OX-LDL was positively correlated with sICAM-1 and sVCAM-1(r=0.729,0.840;P<0.01).ConclusionThe cell adhesion molecules and OX-LDL involves the occurrence and development of AMI,and are closely related with the severity and prognosis of the patients.
