113例鼻咽癌病人放疗前后鼻疾病分析

目的:报道鼻咽癌病人放疗前后鼻疾病的发病率、发病高峰期、临床转归。方法:观察113例鼻咽癌病人放疗前、放疗后半年至3年鼻部的临床资料。结果:放疗前有鼻窦炎30.97%。放疗后鼻窦炎、放射性鼻炎、鼻粘连、萎缩性鼻炎、严重鼻出血的总发病率分别为92.92%、36.28%、29.2%、7.96%、0.88%。在放疗后半年时的发病率及到3年时实际存在的病例,鼻窦炎为79.65%、43.36%,放射性鼻炎为36.28%、0.0%,鼻粘连为23.89%、0.0%,萎缩性鼻炎为0.88%、7.96%。结论:鼻咽癌病人容易并发鼻疾病,放疗前为鼻窦炎,放疗后常见鼻窦炎、放射性鼻炎、鼻粘连,少见萎缩性鼻炎及严重鼻出血。发病高峰期以上3种常见并发症均在半年时。随着时间的推移及治疗的干预,放射性鼻炎、鼻粘连完全缓解,鼻窦炎渐渐减少,萎缩性鼻炎不断发生且治疗无效。

An analysis of nasal diseases in 113 patients with nasopharyngeal carcinoma before and after radiotherapy

Objective: The incidence and the highest time period and changes of nasal diseases in patients with nasopharyngeal carcinoma befor and after radiotherapy were reported. Method: Observing the nasal data of 113 patients with nasopharyngeal carcinoma before radiotherapy and half and 3 years after radiotherapy. Result:The incidence of nasal sinusitis before radiotherapy was 30. 97%. The total incidence of nasal sinusitis , irradiation rhinitis, nasal synechia, atrophic rhinitis, severe epistaxis was 92.92%, 36. 28%, 29.2%, 7.96% and 0.88% respectively after radiotherapy. The incidence in half year after radiotherapy and 3 years after radiotherapy were 79. 65% and 43. 36% in nasal sinusitis respectively; 36.28% and 0.0% in irradiation rhinitis; 23.89% and 0.0% in nasal synechia; 0.88% and 7. 96% in atrophic rhinitis. Conclusion:The patients with nasopharyngeal carcinoma were easy to combine with nasal diseases. The nasal sinusitis was easy to be suffered from before radiotherapy. After radiotherapy, nasal sinusitis, irradiation rhinitis and nasal synechia were often to be seen and the highest time period were in half one year. Atrophic rhinitis and severe epistaxis were few. The irradiation rhinitis and nasal synechia can be cure when the time went by and treatment intervention, the nasal sinusitis could be fewer and fewer but atrophic rhinitis continue happen and cannot be cure.
