摘    要:目的探讨负压封闭引流术(VSD)在创面合并感染性腔隙这类复杂创面处理中的应用效果。方法这类病例首先清创后,VSD充分引流,促进肉芽生长;肉芽创面植皮,VSD敷料覆盖加压,同时感染性腔隙内继续VSD引流,促进肉芽生长。移植皮片成活后,残留腔隙继续VSD敷料引流,直至肉芽填满腔隙,最后通过换药或植皮封闭创面。结果27例患者植皮均一次成活,腔隙经多次VSD引流后填满肉芽,作相应处理后愈合。结论VSD用于创面合并感染性腔隙的处理,操作简单、易行,能明显减轻医师的工作量,患者痛苦小,不需皮瓣或肌皮瓣移植等复杂手术,值得推广应用。

Abstract:Objective To discuss the effects of vacuum sealing drainage (VSD) on the treatment of wound surface combined with infected lacuna. Methods After the debridement, VSD was used for sufficient drainage and facilitate the growth of granulation tissues. Skin grafting was operated on the wound surface of granulation, and VSD was dressed and overlaid. Meanwhile, VSD was also applied to the infected lacuna for drainage in order to facilitate the growth of granulation. After the survival of transplanted flaps, VSD was continuously used in the remained lacuna until the lacuna was filled by the granulation. Eventually, the wound surface was closed by change dressing or skin grafting. Results The skin grafting was carried out at one time in 27 cases. After several times of VSD drainage, the lacuna was filled by granulation and the wound were treated successfully. Conclusion VSD applied to the wound surface combined with infected lacuna is simple and feasible and can significantly reduce doctors'workload and relieve patientsJpain. It is worthwhile to be promoted.
