胸壁放射性溃疡的修复治疗

目的:探讨胸壁放射性溃疡修复的治疗手段。方法:采用创面扩大切除,背阔肌肌皮瓣或局部皮瓣带蒂转移修复,供区植皮术。结果:15例胸壁放射性溃疡采用该法治疗,12例皮瓣全部存活;2例皮瓣边缘与创面愈合欠佳,少量渗出,经多次换药,创面愈合;1例采用带蒂背阔肌肌皮瓣转移修复,皮瓣部分坏死,二次手术采用局部延迟皮瓣转移修复治愈。结论:该法安全,对大创面的修复效果可靠,为一种修复胸壁放射性溃疡的积极有效的方案之一。

The treatment of radiation ulcers of chest

Objective To discuss the treatment methods to repair the radiation ulcers of chest. Methods Uses the injured area expansion excision, transposition and repair of latissimas dorsi musculo cutaneous flap or local skin flap and skin grafting in donor site. Results There were 15 cases of radiation ulcers of chest treated with the method, in which 12 cases were full survival in skin flap;2 were not satisfactorily healed at the edge of skin flap and injured area, followed little effusion. But after the dressing change were performed for several times, the injured area healed.1 case were suffered from necrosis of skin flap partially using transposition of the pedicled latissimus dorsi myocutaneous flap to repair the skin flap, and healed after secondary operation using the delayed local skin flap. Conclusion The method is a good treatment to radiation ulcers of chest as it is safe and efficient.
