单孔腹腔镜阑尾切除术在单病种定额临床路径中的价值

目的 评价单孔腹腔镜阑尾切除术在单病种定额临床路径中的意义.方法 将90例急性阑尾炎临床路径患者分为实验组和对照组,每组各45例.实验组执行单孔腹腔镜阑尾切除术,对照组则行开腹阑尾切除术.对比住院天数、下床时间、住院费用、负变异、满意度情况.结果 实验组住院天数、术后下床时间、临床路径内负变异均显著减少、患者满意度显著提高,以上指标均有统计学差异（P＜0.05）,两组住院费用则无显著差异（P＞0.05）.结论 在单病种定额急性阑尾炎临床路径中单孔腹腔镜阑尾切除术较开腹阑尾切除术更具优势.

The Value of Single Port Laparoscopic Appendectomy in the Clinical Pathway of Single Disease Quota Payment

Objective To evaluate the significance of single port laparoscopic appendectomy in the clinical pathway of single disease quota payment. Methods 90 cases of acute appendicitis patients in clinical pathway were randomly divided into two groups, and 45 cases in each group. Patients in the experimental group were carried Jout single port laparoscopic appendectomy while open abdominal appendectomy in the control group, then compared the aspects such as hospitalization days, time of leaving bed, hospitalization costs, negative variation and satisfaction state. Results Compared with the control group, the hospitalization time, time of leaving bed and negative variation of clinical pathway were decreased significantly, and the satisfaction state of patients also improved obviously, these indexes all had statistical significance （P〈0. 05）, but there was no significant difference in the hospitalization costs between the two groups （P〉0. 05）. Conclusions Single port laparoscopic appendectomy had more advantages than open abdominal appemtectomy in the clinical pathway of single disease quota payment.
