舒芬太尼在老年全髋置换术后自控静脉镇痛的应用

目的：观察老年患者全髋置换术后舒芬太尼静脉自控镇痛的疗效、安全性及副作用.方法：将50例全髋置换术后患者分为舒芬太尼组（S组,n=25）和芬太尼组（F组,n=25）,分别使用舒芬太尼和芬太尼进行术后PCA.记录术后3、6、12、24、48h的镇痛效果.记录疼痛评分、副作用、生命体征及是否使用其它镇痛药物.结果：术后12h安静痛VAS及48h时需求按压及有效按压次数S组少于F组（P〈0.05）,两组效果均满意,S组满意率高于F组（P〈0.05）.结论：舒芬太尼可安全有效地用于老年患者全髋置换术的PCIA,镇痛作用确切,等效剂量舒芬太尼镇痛,镇静效果强于芬太尼.

Patient--controlled intravenous analgesia with sufentanil in elderly patients after total hip replacement

Objective： To investigate the clinical efficacy, safety and complication of patient--controlled intravenous analgesia in elderly patients after total hip replacement. Methods：Fifty patients underwent after total hip replacement were randomly divided into two groups of PCIA with sufentanil （group S, 25cases）and PCIA with fentany（group F, 25cases）. The visual analogue scale（VAS）, side effects and satisfaction were recorded at 3.6. 12.24.48h during PCIA. Results：There were no significant differences in patient satisfaction between two groups. VSA and requested does of sufentanil at some time points were less in group S than those in group F（P〈0. 05）. The percentage of excellent satisfaction of the patients in group S was higher than that in group F. Conelusion：Sufentanil could be safely and effectively used in PCLA after total hip replacement. Sufentanil has better anal- gesic effect in comparison with fentany. The effect of analgesia and sedation of sufentanil is better than that of fentany when equivalent dose of sufentanil and fentany is used.
