妇科腹腔镜手术麻醉体会

摘    要:目的:总结腹腔镜妇科手术麻醉处理的临床经验.方法:选择腹腔镜妇科手术患者200例,随机分为连续硬膜外麻醉组和气管插管加静脉复合麻醉组,对麻醉前,气腹后5min,改头低脚高位后5min及排出腹腔气体平卧后5min平均动脉压(MAP)、心率(HR)、脉搏氧饱和度(SpO2)进行了回顾性分析.结果:气腹后5分钟、改变体位5分钟时,两组HR均较麻醉前明显增快(P〈0.05) MAP变化不明显 连续硬膜外麻醉后脉搏SpO2有所下降,气管插管加静脉复合麻醉组则无此现象,组间比较有显著性差异(P〈0.05).结论:腹腔镜妇科手术气管插管加静脉复合麻醉优于硬膜外麻醉.

Abstract:Objective:Summary peritoneoseope gynecology department surgery anaesthesia processing clinical experience. Methods:The choice peritoneoscope gynecology department surgery patient 200 examples, divide into outside the hard membrane to anaesthetize the group and the trachea intubation continuously stochastically add the vein compound anaesthesia group before, to anaesthetizes, after the pneumoperitoneum 5min, after changed a low top digit 5min and discharges abdominal cavity gas horizontal 5rain average arterial pressure (MAP) ,heart rate (HR) ,the pulse oxygen degree of saturation (SpO2) has carried on the review analysis. Results:When pneumoperitoneum latter 5 minute, change body posture 5 minutes before,two group of HR anaesthetizes increases obviously quickly (P〈0.05) ;The MAP change is not obvious;Outside the hard membrane anaesthetizes pulse SpO2 to have the drop after continuously,the trachea intubation adds the vein compound anaesthesia group not to have this phenomenon, the group the comparison has the significance difference (P〈0.05). Conclusion: The peritoneoscope gynecology department surgery trachea intubation adds the vein compound anaesthesia to surpass outside the hard membrane to anaesthetize.
