摘    要:目的探讨超声引导下麦默通系统在乳腺外科中的应用价值。方法262例共461个经临床及超声显像检查明确为乳腺肿块的患者,采用8G穿刺针在超声引导下行肿块切除术或活检术,术后1~3个月后复查。结果所有患者均在实时超声引导下完成肿块切除术,超声引导成功率达100%,术后近期并发症发生率5.73%,并发症以创面血肿、皮肤青紫和术中出血为主,另有肿块残留2例。无1例有明显手术疤痕。结论超声引导下麦默通装置具有定位准确、微创切除或活检乳腺肿块的功能,尤其适合于直径在3cm以下的肿块。

Abstract:Objective To evaluate the clinical application of ultrasound guided Mammotome minimally invasive system in breast surgery. Methods 461 breast masses from 262 patients were identified by clinical examination and ultrasonic imaging method. Excision or biopsy of the lesions were performed using 8G Mammotome probe under ultrasonic guidance, and then all cases were followed up in 1-3 month. Results All lesions were excised under real-time ultrasound guidance, and the probability of success was 100%. The postoperative complication rate in recent was 5.73%. Most of the complications were ecchymoma, skin cyanosis and operation bleeding, and mass residue was found in two cases, no apparent scar was observed in all patients. Conclusions Mammotome system guided by ultrasound is position fixed accurately and minimally invasive to resect or biopsy breast mass, especially suitable for less than 3cm in size.
