中药熏蒸辅助治疗对重度类风湿膝关节炎人工膝关节置换术后患者康复效果的影响

目的:探讨中药熏蒸辅助治疗对重度类风湿膝关节炎人工膝关节置换术后患者康复效果。方法:选取我院骨科2017年1月至2018年2月收治的重度类风湿膝关节炎需行人工膝关节置换术患者83例。按就诊先后顺序分为试验组和对照组,试验组40例,对照组43例。对照组患者常规康复功能锻炼,试验组患者在对照组的基础上给予中药熏蒸辅助治疗,治疗3个月。对比两组患者治疗治疗前和治疗后后血清因子水平,治疗前后生活质量评分量表(SF-36)评分,两组患者骨关节炎指数评分(The Western Ontario and Mcmaster Universities Osteoarthritis Index,WOMAC)、膝关节评分(Hospital for special surgery,HSS)。结果:经治疗治疗前和治疗后后试验组血浆纤维蛋白(FIB)、D-二聚体、IL-1β、白细胞介素8(IL-8)水平均明显降低(P<0.05);试验组SF-36量表评分显著高于对照组(P<0.05);试验组WOMAC评分、HSS评分显著高于对照组(P<0.05)。结论:中药熏蒸辅助治疗能有效提高重度类风湿膝关节炎人工膝关节置换术后患者康复效果。

Effect of Fumigation Assisted by Traditional Chinese Medicine on Rehabilitation of Patients with Severe Rheumatoid Knee Arthritis after Artificial Knee Replacement

Objective:To explore the rehabilitation effect of Chinese medicine fumigation assisted treatment on patients with severe rheumatoid knee arthritis after artificial knee replacement.Methods:83 patients with severe rheumatoid knee arthritis who needed to undergo artificial knee replacement were selected from our department of orthopedics from January 2017 to February 2018.According to the order of visits,they were divided into test group and control group,40 cases in test group and 43 cases in control group.The patients in the control group were given routine rehabilitation functional exercises,and the patients in the test group were given Chinese medicine fumigation adjuvant therapy on the basis of the control group for 3 months.Comparing the serum factor levels before and after treatment in two groups of patients,the quality of life score scale(SF-36)before and after treatment,the osteoarthritis index score(The Western Ontario and Mcmaster Universities Osteoarthritis Index,WOMAC)of the two groups of patients,Knee score(Hospital for special surgery,HSS).Results:The levels of plasma fibrin(FIB),D-dimer,IL-1β,and interleukin 8(IL-8)in the experimental group were significantly reduced before and after treatment(P<0.05);the experimental group The SF-36 scale score was significantly higher than the control group(P<0.05);the test group WOMAC score and HSS score were significantly higher than the control group(P<0.05).Conclusion:Chinese herbal medicine fumigation can effectively improve the rehabilitation effect of patients with severe rheumatoid knee arthritis after artificial knee replacement.
