摘    要:目的用定量组织多普勒成像(QTVI)技术测算Tei指数评价心肌梗死(MI)患者左心功能。方法应用脉冲血流多普勒方法测量30例正常人和30例MI患者二尖瓣关闭至下一次开放的时间(a)、主动脉射血时间(b),计算传统Tei指数(c-Tei);QTVI条件下,测算二尖瓣环各位点的Tei值及其均值(t-Tei);所有MI患者行同位素门控心血池造影检查,记录左心室射血分数(ECT-EF)、左心室局部射血分数(REF)及左心室峰充盈率(PFR)。结果(1)心肌梗死组(MI)患者左室整体t-Tei较正常对照组明显延长;且不同部位的心肌梗死患者,其各室壁Tei值延长的程度不同;(2)t-Tei值与c-Tei值有密切的相关性;MI患者t-Tei值与LVEF、门控心血池造影所测得的ECT-EF、PFR之间呈明显的负相关性,二尖瓣环各位点的Tei指数与REF之间呈负相关关系。结论利用QTVI测算Tei指数可准确评价心脏整体和局部功能,有临床推广应用价值。

Abstract:Objective To assess the left ventricular function of myocardial infarction (MI) patients using Tei index via quantitative tissue velocity imaging (QTVI).Methods Thirty MI patients (MI group) and thirty healthy subjects(Con group) were involved.Tei index was defined as (a-b)/b.By pulsed blood Doppler,a was the interval between the end and onset of the mitral inflow,and b was the ejection time of the artery.By QTVI,a and b were measured at six different sites of the mitral annulus.The mean value of the six different sites(t-Tei) was calculated.All MI patients underwent multiple gated cardiac blood pool imaging examination to obtain LV global ejection fraction (ECT-EF),regional ejection fraction (REF) and peak filling rate (PFR).Results 1.The value of t-Tei index in the MI group was longer than that of the con group and there were different Tei values in different walls of MI;2.In MI group,there was a significant correlation between t-Tei and Tei value on pulse blood Doppler.Meanwhile,the negative correlation of t-Tei with ECT-EF,PFR and the negative correlation of the regional Tei index with REF were good.Conclusions It has clinical application in evaluating left ventricular global and regional function correctly using Tei index by quantitative tissue velocity imaging (QTVI).
