腹腔镜与开腹手术治疗急性粘连性肠梗阻的疗效对比研究

目的:探讨分析腹腔镜与开腹手术治疗急性粘连性肠梗阻的临床疗效。方法:回顾分析2014年5月至2016年5月收治的90例急性粘连性肠梗阻患者的临床资料,分为开腹组与腹腔镜组,每组45例,对比两组患者围手术期指标、并发症发生率及血清前白蛋白、C反应蛋白、降钙素原。结果:与开腹组相比,腹腔镜组术中出血少,胃肠功能恢复时间、下床活动时间、拔除导尿管时间、住院时间缩短,VAS评分降低,并发症发生率低,差异有统计学意义(P0.05)。与术前相比,两组患者术后1周血清前白蛋白、C反应蛋白及降钙素原水平降低,两组间相比腹腔镜组上述指标改善更加显著,差异有统计学意义(P0.05)。结论:腹腔镜手术治疗急性粘连性肠梗阻对周围组织创伤小,术后康复快,并发症发生率低,临床应用价值突出。

Comparative effectiveness research of laparoscopic surgery and open surgery in the treatment of acute adhesive intestinal ob-struction

Objective:To explore and analyze the clinical curative effect of laparoscopic surgery and open surgery in the treat -ment of acute adhesive intestinal obstruction .Methods:Clinical data of 90 patients with acute adhesive intestinal obstruction were retro-spectively analyzed .All the patients were divided into two groups:the open group and laparoscopic group with 45 cases in each group . The perioperative indicators ,the incidence of complications and serum prealbumin , C-reactive protein and procalcitonin of two groups were compared.Results:The intraoperative blood loss,gastrointestinal function recovery time,ambulation time,time of the catheter re-moval,hospital stay,VAS score,incidence of complications of the laparoscopic group were less than those of the open group with statisti -cally significant difference (P<0.05).The serum prealbumin,C-reactive protein and procalcitonin levels in two groups in 1 week after surgery were lower than those before surgery ,and those in laparoscopic group were significantly better than those in open group ,with sta-tistically significant difference (P<0.05).Conclusions:Compared with open surgery,laparoscopic surgery for acute adhesive intestinal obstruction has less damage on the surrounding tissue ,faster recovery and lower complications ,the clinical value is outstanding .
