卵巢畸胎瘤的CT表现及其病理基础

目的 探讨卵巢畸胎瘤的CT表现特点及其病理基础，以提高CT诊断的准确性。方法 回顾性分析我院经手术病理证实的18例19个卵巢畸胎瘤的CT影像资料，结合病理讨论其CT表现特点。结果 本组19个畸胎瘤中，17个见脂肪密度区。16个成熟囊性畸胎瘤中，14个边缘光滑，11个有浮球征或畸胎结节，1个见脂液平面，12个见囊壁钙化。2个恶变成熟性畸胎瘤均为囊实性肿块，囊壁不规则增厚，肿块边缘不规则，囊内见条索状厚间隔和散在钙化及小片状脂肪密度影。1个未成熟畸胎瘤为实质性肿块，边缘浅分叶状，内见散在小钙化灶和脂肪密度影以及血曲带状稍低密度影，后者镜下为脑组织。结论 卵巢畸胎瘤CT能作出正确诊断，囊壁不规则增厚和／或囊内出现厚间隔是其恶变的征象。实质性肿块伴有脂肪和／或钙化影为未成熟畸胎瘤重要征象。

The CT diagnosis and pathologic basis of ovarian teratoma

Objetive To study (he CT feature and pathologic basis of ovarian leratoma and to nnplove the CT diagnostic accuracy. Methods 18 cases(19 tumors) with ovarian teratoma were selected, all patientes were surgically and pathologically proved alter CT scan. The characteristics of their images were discussed correlated with their pathology. Results Fat attenuation were lonnd in 17 ol 19 tumor. In 16 benign mature evstic teraloma 14 cases were with smooth border, I 1 patients had a round mass ol teratonodnle in the cyst, 1 patients had tat-fluid level, 12 patients had calcification, 2 malignant mature cystic teratoma display cyst-solid masses, wilh irregular border and thickness, endocystic projections and calcfication,fat attenuation were also found in the cyst. 1 miniature teratoma presented as solid mass with irregular border and calofieation. Nome curved band-like lower attenuation areas were also lonnd on CT, which were brain lissue confirmed by pathology. Conclusion Ovarian teratoma had their characteristic of CT feature. They could be diagnosed accurately by CT Irregular border and thickness of the cyst wall are their malignant sign. The solid mass with fat attenuation and calcification is the important sign of immature teratoma Some curved band -like lower attenuation is another important sign of it.
