C型臂下椎间盘旋切术治疗腰椎间盘突出症的疗效观察

目的对C臂引导下椎间盘旋切术治疗腰椎间盘突出症的中远期疗效观察,对其安全性进行动态评估。方法 32例符合椎间盘旋切术适应证的腰椎间盘突出症患者,在0.5%利多卡因局麻下,以C臂X线机透视引导穿刺针至正确位置,行椎间盘旋切术。做好围术期护理及术后随访观察效果,随访时间为12个月。结果参照改良Macnab标准,优17例(53.13%),良11例(34.37%),可2例(6.25%),差2例(6.25%)。术后一过性穿刺部位疼痛8例,皮下血肿2例,未出现严重不良反应病例。结论 C臂引导下椎间盘旋切术治疗腰椎间盘突出症疗效确切,但应加强围术期护理以策安全,并加强医护患之间的沟通协调,提高患者满意度。

Evaluation of the efficacy of percutaneous discectomy with dekompressor on protrusion of lumber intervertebral discs under C-armed fluoroscopy

Objective To evaluate the effects of percutaneous discectomy with dekompressor to cure sciatica caused by protrusion of lumber intervertebral discs under C-armed fluoroscopy.Methods 32 Patients diagnosed clearly as sciatica caused by protrusion of lumber intervertebral discs were enrolled.After anesthetized with 0.5% lidocaine,placing the needle into the correct position under C-armed fluoroscopy,then percutaneous discectomy was performed with dekompressor.Perioperative nursing was paid attention during the whole period.The follow up was made until 12 months after operation.Results According to the modified Macnab outcome,17 cases(53.13%) were excellent,11 cases(34.37%) good,and 2 cases(6.25%) fair,2 cases(6.25%) were poor.No severe complications were found.Conclusion Percutaneous discectomy performed with dekompressor is a safe and effective method to cure sciatica caused by protrusion of lumber intervertebral discs,meanwhile,studious perioperative nursing as well as close communications among doctors,nurses and patients help increase the efficacy and enhance the degree of satisfaction.
