肠内营养在腹部手术后胃瘫治疗中的应用分析

目的探讨肠内营养在腹部手术后胃瘫治疗中的作用。方法对2000年至2003年在我院收治的腹部手术后胃瘫患者78例进行回顾性分析,其中肠内营养(EN)组30例,肠外营养(PN)组30例及对照组18例。检测治疗前后的营养指标,并比较各组胃肠减压量、胃瘫恢复时间及并发症发生率。结果EN组、PN组治疗后体重(BW)、前白蛋白(PA)和白蛋白(Alb)均高于对照组(P<0.05),而EN组与PN组间差异无统计学意义;EN组胃排空恢复时间明显短于PN组和对照组(P<0.01);胃肠减压量在治疗7 d后明显少于PN组和对照组(P<0.05),治疗14 d后与PN组和对照组差异有统计学意义(P<0.01)。EN组并发症的发生率明显低于PN组和对照组。结论肠内营养能有效促进胃瘫的恢复,对腹部手术后胃瘫患者应尽量采用肠内营养支持。

Enteral nutrition in the treatment of gastroparesis after abdominal operation

Objective To explore the role of enteral nutrition(EN) in the treatment of gastroparesis after abdominal operation.Methods The clinical data of 78 patients were analyzed retrospectively.Among them,30 cases were in the EN group,30 cases were in the parenteral nutrition(PN) group,and the remaining 18 cases were in the control group.Nutritional parameters were evaluated before and after the treatment.Gastrointestinal decompression amount,recovery time of gastroparesis and morbidity among the three groups were compared.Results The levels of BW,PA and Alb in EN or PN group were significantly higher than those in control group(P<0.05) and no statistical difference was found between EN and PN groups.The restoring time of gastroparesis,gastrointestinal decompression amount and incidence of complications were significantly decreased in EN group as compared with PN or control group.Conclusion EN can treat gastroparesis and reduce complications of the patients with gastroparesis after abdominal operation more effectively.
