16666例灾区板房医院住院患者医院感染调查分析

目的了解地震灾区过渡板房医院住院患者医院感染的现状与特点,分析发生的影响因素,评价防控效果。方法采用前瞻性与回顾性调查相结合的方法 ,对收治的16666例患者医院感染情况进行调查分析。结果 2009年1~12月16666例患者发生医院感染555例,医院感染率3.33%;Ⅰ类手术切口感染率0.4%;病原学检测率54.81%;高危科室是ICU、骨科、感染疾病科、神经外科、胸外肿瘤科;感染部位以呼吸道为主,其次是泌尿道、手术部位;病原菌以革兰阴性菌为主。医院感染与收治的病种、病情、年龄、住院环境、侵袭性操作、抗菌药物合理使用、病原学检测率、消毒灭菌效果监测、医院感染管理团队执行力等有关。结论在地震灾区过渡板房医院的特殊环境条件下,医院感染管理必须与恢复医疗救治水平和完善医疗质量安全管理同时启动,及时准确客观评估,采取有效措施,科学规范管理,重点监测,增强医院感染管理的团队执行力。

16666 cases of disaster board room patient in a hospital analysis of nosocomial infection

Objective To understand the earthquake-stricken areas the transition board room patient in a hospital status and characteristics of nosocomial infection,to analyze the impact of factors to evaluate prevention and control effectiveness.Methods A prospective and retrospective investigation of a combination of methods,16666 cases of patients admitted to the hospital infection investigation and analysis.Results In January 2009 to December 16666 cases of nosocomial infection in 555 cases occurred in patients,the hospital infection rate of 3.33%;I type of incision infection rate of 0.4%;pathogen detection rate of 54.81%;high-risk departments are ICU,orthopedics,infectious diseases Section,neurosurgcry,thoracic oncology;the site of infection with respiratory mainly.Followed by urinary tract,surgical site;pathogens were mainly Gram-negative bacteria.Hospital infection and admitted to the diseases,condition,age,hospital environment,invasivc operation,the rational use of antimicrobial agents,etiology detection rate,disinfection and sterilization effect monitoring,hospital infection management team and other relevant executive power.Conclusion Transition board in the earthquake affected areas of special environmental conditions in the hospital room,the hospital infection management must be to restore the level of medical care and improve health care quality and safety management at the same time start,timely and accurate and objective assessment,and take effective measures,scientific and standardized management,focus on monitoring,to increase hospitals infection control team execution.
