| 摘 要： | 目的:观察益肾平肝方(熟地、杜仲、桑寄生、钩藤、罗布麻、葛根和川牛膝)对高血压早期肾损害病人动态血压、肾功能及血管内皮功能的影响。方法:收集高血压早期肾损害患者90例,随机分为3组,中药组(益肾平肝方),中西药结合组(益肾平肝方+贝那普利),西药组(贝那普利),用药3个月,观测治疗前后的24h动态血压、尿微量白蛋白(mALB)、尿β2-微球蛋白(β2-MG)、血清内皮素(ET)及一氧化氮(NO)的变化。结果:中药组治疗后动态血压各指标与治疗前比较均有显著下降(P0.01～0.05),3组组间比较无显著差异;中药组能明显降低尿mALB及β2-MG水平(P0.01),且略优于西药组;治疗后ET、NO两指标中药组治疗前后比较有显著差异性(P0.01),中药组、中西药结合组与西药组组间比较差异显著(P0.01)。结论:益肾平肝方可有效降低原发性高血压早期肾损害患者的白昼和夜间血压,并在一定程度上可改善和逆转异常的血压昼夜节律;能明显降低患者的微量蛋白尿,该药的降压及肾保护作用可能是通过影响血管内皮功能实现的。
