阻挡钉在下肢长骨干骺端骨折中的应用价值

目的评价阻挡钉技术在长骨干骺端骨折中的应用价值。方法将本院2009—2012年收治的54例长骨干骺端骨折患者分为两组,每组27例,观察组采用阻挡钉加强交锁髓内钉固定治疗,对照组采用非阻挡钉交锁髓内钉治疗。比较两组患者的愈合指标、术后并发症发生率及关节功能恢复情况。结果观察组平均手术时间为(97.5±10.3)min,对照组平均手术时间为(104.6±11.2)min,观察组手术时间明显缩短(P<0.05)。观察组术后并发症发生率为11.10%,对照组并发症发生率为37.02%,与对照组相比,观察组术后并发症发生率大大降低(P<0.01)。同时,观察组关节功能恢复优良率为88.88%,对照组优良率为62.95%,观察组关节功能恢复明显优于对照组(P<0.05)。结论阻挡钉加强交锁髓内钉治疗长骨干骺端骨折能有效增加骨折端稳定性,固定牢靠,创伤小,患处关节功能恢复良好,术后并发症少,在长骨干骺端骨折治疗中疗效较好。

Application of blocking nails to long bone metaphyseal factures of lower extremities

Objective To discuss the application of blocking nails to long bone metaphyseal factures of lower extremities.Methods A total of 54 cases of long bone metaphyseal factures in lower extremities,which were treated from 2009 to 2012,were divided into trial group and control group(n=27/group).The trial group was treated with interlocking intramedullary nails plus blocking screws,and the control group with interlocking intramedullary nails alone.Results The operative time was(97.5±10.3) min in the trial group and(104.6± 11.2) min in the control group,between which there existed a significant difference(P<0.05).The rates of postoperative complication were 11.10%(trial group) and 37.02%(control group)(P<0.01).At the same time,the recovery rate of joint function of the trial group was higher than that of the control group(88.88% vs 62.95%,P<0.05).Conclusion The application of blocking nailing to the treatment of long bone metaphyseal factures in lower extremities has an advantage in simple operation,stable fixation and good recovery of joint function.
