立体定向放射配合全脑放疗治疗脑转移瘤的疗效观察

摘    要:目的对脑转移瘤患者采用立体定向放射治疗配合全脑放疗,观察其疗效。方法对31例脑转移瘤患者给予立体定向放射配合全脑放疗,立体定向放射治疗剂量为16-30 Gy/2-6次,或单次照射剂量10-12Gy;全脑放疗剂量为30-40 Gy/15-20次。结果除1例患者无症状和阳性体征外,其余30例患者中放疗后神经系统症状和体征完全缓解12例,部分缓解18例。放疗结束后影像学疗效:完全缓解(CR)为32.3%,部分缓解(PR)为54.8%,无变化(NC)为12.9%,患者的卡氏评分提高率为100%,中位生存期为8.5个月。结论立体定向放射治疗配合全脑放疗治疗脑转移瘤能有效的控制脑转移病变,提高患者的生活质量并延长生存期。

Abstract:Objective To observe the early effects of stereo tactic radiotherapy combined with whole brain radiotherapy(WBRT)for 31 patients with brain metastasis.Methods 31 patients with brain metastasis were treated by stereo tactic radiotherapy combined with WBRT. The dose of stereo tactic radiotherapy was 16-30Gy/2-6 times or 10-12Gy single time.The dose of WBRT was 30-40Gy /15-20 times.(Results) The significant remission rate of signs and symptoms in 30 patients was 40%,partial remission rate was 60%.Within 1-2 months after radiotherapy,the CR of imaging was 32.3%,PR was 54.8%,and NC was 12.9%.The median survival was 8.5 months.Conclusion Stereo tactic radiotherapy combined with WBRT for brain metastasis could control the lesions of brain effectively,improve life quality and prolong survival.It has few side effects.
