VEGF-C、MMP-2的表达与人胃癌淋巴转移

摘    要:目的:检测VEGF-C、MMP-2在正常胃粘膜及胃癌中的表达情况以及它们之间的相关性,探讨胃癌发生淋巴结转移的机制。方法:应用免疫组化SABC法检测VEGF-C、MMP-2在正常胃粘膜及胃癌组织中的表达情况。结果:VEGF-C在正常胃粘膜中阴性表达,在胃癌组织中表达阳性率为44.4%(P<0.01)。MMP-2在正常胃粘膜细胞中阳性表达率为15.0%,在胃癌组织中的表达阳性率为54.0%(P<0.01)。在高、中分化组和低分化组中,VEGF-C蛋白的阳性表达率分别为29.6%和55.6%(P<0.05),MMP-2蛋白的阳性表达率分别为37.0%和66.7%(P<0.05)。在未及浆膜层和浸润浆膜层两组中,VEGF-C的阳性表达率分别为22.7%和56.1%(P<0.05),MMP-2的阳性表达率分别为27.3%和68.3%(P<0.05)。在有淋巴结转移组和无淋巴结转移组,VEGF-C的阳性表达率分别为64.5%和25.0%(P<0.05),MMP-2的阳性表达率分别为77.4%和31.3%(P<0.05)。VEGF-C阳性表达与MMP-2的阳性表达呈正相关。结论:VEGF-C、MMP-2的阳性表达与胃癌的组织学分级、浸润深度、淋巴结转移密切相关。VEGF-C、MMP-2在胃癌中的过表达可能参与了胃癌的淋巴道转移。VEGF-C的阳性表达与MMP-2的阳性表达呈正相关,提示VEGF-C可能通过增强MMP-2的表达,提高肿瘤细胞的侵袭性和转移能力。

Abstract:Objective:To investigate the possible mechanism of the lymphatic metastasis of gastric carcinoma and the clinicopathological significance of VEGF-C and MMP-2 expression in gastric carcinoma.Methods: An immunohistochemical SABC(Strept Avidin Biotin Complex) method was used to examine the expression of the two kinds of proteins.Results: In normal gastric mucosa,staining for VEGF-C protein was negative,in gastric carcinoma its positive rate was 44.4%(P<0.01).In normal gastric mucosa,positive staining for MMP-2 protein was 15.0%,in gastric carcinoma was 54.0%(P<0.01).In high/moderate differentiation group and poor differentiation group,the positive rates of VEGF-C were 29.6% and 55.6% respectively(P<0.05),the positive rates of MMP-2 were 37.0% and 66.7% respectively(P<0.05).In serosa infiltration group and without serosa infiltration group,the positive rates of VEGF-C were 56.1% and 22.7% respectively(P<0.05),the positive rates of MMP-2 were 68.3% and 27.3% respectively(P<0.05).In lymph node metastasis negative group and lymph node metastasis positive group,the positive rates of VEGF-C were 64.5% and 25.0% respectively(P<0.05),the positive rates of MMP-2 in the two groups were 77.4% and 31.3% respectively(P<0.05).A positive correlation was observed between the expression of VEGF-C and that of MMP-2(P<0.05).Conclusion: The expressions of VEGF-C,MMP-2 were associated with lymph node metastasis,the histological grade and the depth of gastric carcinoma invasion. Over expression of the two proteins may participate the lymph node metastasis of gastric carcinoma.The positive correlation between the expression of VEGF-C and that of MMP-2 indicates that over expression of VEGF-C can enhance the aggressive behavior of gastric carcinoma,and this may be very helpful for the gastric carcinoma cells to metastasis to the lymph nodes.
