外周动静脉同步换血治疗新生儿高胆红素血症

目的　探讨外周动静脉同步换血对新生儿高胆红素血症的疗效。方法　在多功能心电监护仪检测下 ,行外周动脉抽血 ,静脉输血 ,记录换血出入量 ,换血前后作胆红素、血常规、生化、血气和血培养等检查。结果　换血时间 2 .4～ 3 .8h。换血后血清总胆红素、结合和非结合胆红素水平明显下降 (P <0 .0 5和 0 .0 1 )。血红蛋白和血糖明显增高 (p <0 .0 5) ,无明显副作用。结论　外周同步换血疗法简便易行、副作用少 ,安全有效 ,值得临床推广应用。

Treatment of Peripheral Arterio-Venous Exchange Transfusion for Neonatal Hyperbilirubinemia

Objective To investigate the effect of peripheral arterio-venous exchange transfusion on neonatal hyperbilirubinemia. Methods Being in ward to the multi-function electrocardio-monitor, the patients were treated by arterio-venous exchange transfusion. The intake and output volume were recorded. The serum total bilirubin, blood routine examination, blood biochemistry examination, blood gas analysis were examined. Results Time of exchange transfusion was 2.4 to 3.8 hours. After exchange transfusion, serum total bilirubin, conjugated bilirubin and unconjugated bilirubin were decreased (P<0.05 or P<0.01). Levels of ferrohemoglobin and blood sugar were increased without side effect (P<0.05). Conclusions Peripheral arterio-venous exchange transfusion is a simple, safe and effective method for neonatal hyperbilirubinemia with little side effect.
