妊娠高血压综合征患者的多焦视网膜电图表现

目的研究妊娠高血压综合征(妊高征)患者的多焦视网膜电图(mfERG)及其与病情、预后的关系。方法正常对照组31例,妊高征组77例,并分为轻度、中度、重度。对所有对象行mfERG检查,分析其与妊高征程度、视网膜病变程度、母婴预后、产后随访结果的关系。结果不同程度的妊高征患者及不同程度视网膜病变的妊高征患者的mfERG表现均出现异常(P<0.05)。轻度妊高征患者即出现1、2环的视网膜反应密度降低(P<0.05)。mfERG出现异常早于视网膜出现病变(P<0.05)。与mfERG正常的相比,mfERG异常的妊高征的母婴预后较差(P<0.05)。妊高征患者产后mfERG恢复迟于视网膜病变的恢复。结论观察妊高征患者的mfERG表现有助于更好地监测病情、判断预后、指导治疗。

mfERG findings of pregnancy-induced hypertension syndrome

Objective To study the multifocal electroretinogram ( mfERG) changes of pregnancy-induced hypertension syndrome(PIH) and its relationship to prognosis. Methods MfERG was recorded in 31 normal subjects and 77 patients with mild, moderate and severe PIH. The average response densities of first-order kernel ( FOK) responses of mfERG from 5 rings were analyzed. Results The mfERGs in various stages of PIH and retinopathy were significantly abnormal compared to those in normal subjects (P < 0, 05 ). The response densities of 1 and 2 rings in the mild stage of PIH were significantly lower than those in normal subjects ( P < 0. 05 ) . and that of 1 ring in PIH patients without retinopathy was significantly lower than those in normal subjects ( P < 0. 05). Reduction degrees of average response and response densities from 5 rings were dependent on the severity of PIH and retinopathy (P < 0. 05 ). The worse prognosis results were seen in patients with abnormal mfERG and their neonatal ( P < 0. 05 ). Recover of the symptoms of retinopathy in PIH patients was prior to that of mfERGs responses. Conclusion MfERGs is a useful tool for the observation of stage, evaluation of prognosis and monitor of treatment in PIH patients.
