摘    要:目的探讨全直肠系膜切除术(TME)在中、低位直肠癌治疗的应用。方法回顾总结129例行全直肠系膜切除术的中、低位直肠癌病例,分析疗效、局部复发和术后并发症。结果围手术期死亡率1.55%(2/129);5年生存率66.70%(58/87);2年局部复发率6.20%(8/129);发生吻合口瘘和吻合口狭窄分别为12例(9.30%)和10例(7.75%)。结论全直肠系膜切除术可有效降低局部复发率,减少术后并发症,可提高病人生活质量和生存率,是治疗中、低位直肠癌的理想手术。

Abstract:[Objective] To investigate the application of total mesorectal excision (TME) for middle and low rectal cancer. [Methods] The clinical data of 129 cases with middle and low rectal cancer treated by TME were retrospectively analyzed including the therapeutic effect, local recurrence, and complications. [Results] Perioperative death rate was 1.55% (2/129); 5-year survival rate was 66.70% (58/87); 2-year local recurrence rate was 6.20% (8/129). The cases with anastomotic leakage and anastomotic constriction was 12 (9.30%) and 10 (7.75%) respectively. [Conclusion] Total mesorectal excision is a perfect operation for middle and low rectal cancer to reduce local recurrence, decrease postoperative complications, and improve postoperative quality of life of the patients and survival rate.
