鼻咽癌调强放疗与三维适形放疗口腔反应临床分析

目的观察鼻咽癌调强放疗与三维适形放疗的口腔黏膜及口干反应。方法将本院收治的82例鼻咽癌患者随机分为2组,每组41例,其中观察组行调强放疗,对照组行三维适形放疗,比较2组患者治疗后的口干及口腔黏膜反应。结果观察组口腔黏膜反应多为Ⅰ、Ⅱ级,少数为Ⅲ级反应,口干反应多为Ⅰ、Ⅱ级,未出现Ⅳ级反应;对照组的口腔黏膜反应多为Ⅱ、Ⅲ级,口干反应也同样多为Ⅱ、Ⅲ级。观察组的黏膜反应、口干明显轻于对照组(P<0.05)。结论调强放疗与三维适形放疗治疗鼻咽癌均可获得满意疗效,但调强放疗较三维适形放疗口腔毒副反应轻,更为安全,且缩短住院时间,值得临床推广应用。

Clinical analysis of oral reaction of intensity modulated radiotherapy and three-dimensional conformal radiotherapy for nasopharyngeal carcinoma

Objective To observe nasopharyngeal carcinoma reaction of intensity modulated radiation therapy and three-dimensional conformal radiotherapy for oral mucosa reaction and xerostomia.Methods Eighty-two nasopharyngeal carcinoma patients in our hospital were randomly divided into two groups,41 in each group.The observation group received intensity modulated radiation therapy,while the control group underwent three-dimensional conformal radiotherapy.Xerostomia and oral mucosa reactions of the two groups after treatment were compared.Results Most of the oral mucosa reactions of the observation group were grades Ⅰ and Ⅱ.Only few of them were grade Ⅲ.Most of xerostomia reactions were gradesⅠ and.Grade Ⅳ reactions were not observed.Most of the oral mucosa reactions of the control group were grades Ⅱ and Ⅲ.Most of the xerostomia reactions were grades Ⅱ and Ⅲ.The oral mucosa reactions and xerostomia were lighter in the observation group than in the control group(P<0.05).Conclusion Both intensity modulated radiotherapy and three-dimensional conformal radiotherapy for nasopharyngeal carcinoma bring satisfactory curative effects.But the intensity modulated radiotherapy may have lighter oral toxicity,more safety,and shorter hospital stay.Thus it is worthy of clinical application.
