不可复性盘前移位致颞下颌关节的病理组织学研究

摘    要:目的:研究盘前移位对兔颞下颌关节组织的影响。方法:对6只成年新西兰大耳白兔的右侧颞下颌关节分别切除盘前及内外侧韧带,然后将关节盘前拉,用丝线固定于颧弓部,造成不可复性盘前移位,左侧颞下颌关节为非手术对照。术后4周处死动物,解剖显微镜下切除关节组织,常规处理,石蜡包埋,HE染色。结果:手术侧关节髁突表面纤维组织呈局灶性增厚,软骨细胞增生活跃,关节出现大量新生的软骨细胞,核大,深染或双核,双板区纤维组织排列紊乱,对照关节则无组织学异常表现,结论:不可复性盘前移位可造成兔颞下颌关节形态及组织学变化,其早期组织学特点为髁状突的进行性改建与关节盘及双板区的退行性改建并存。

Abstract:Objective To study the effect of disc displacement on temporomandibular joint. Methods The right disc displacement was created by incising the lateral, medial and anterior attachment and anteriorly fixing the disc to the zygomatic arch in 6 New Zealand rabbits. The left joint served as a control. The animals were killed at 4 weeks and the craniomandibular joint tissues were excised under dissecting microscope. Results Histologic examination showed that all 6 joints under disc displacement had evidence of histopathological changes including irregular fibrous thickening of the articular soft tissue cover, chondrocyte proliferation, neonatal chondrocyte with large nucleus, double neclei in the disc and disordered collager fiber in the posterior attachment. No histologic changes were observed in the control joint. Conclusions The results suggest that surgically induced disc displacement causes histologic changes and form changes of temporomandibular joint in rabbits, which is characterized by progressive remodeling in the condylion and degenerative changes in the disc and posterior attachment in the early period.
