腹腔镜在普外科急腹症诊断和治疗中的应用研究

目的探讨腹腔镜在普外科急腹症诊断和治疗中的临床应用效果。方法回顾性分析2008年3月～2012年3月入住笔者所在医院普外科的120例急腹症患者的临床资料,将本组患者随机分为对照组与观察组,对照组(n=60)采用急腹症剖腹探查,观察组(n=60)采用腹腔镜探查。对上述两组患者手术时间、住院时间、术中出血情况、探查结果进行对比分析。结果观察组与对照组手术时间比较,差异无统计学意义(P>0.05);观察组术中出血量明显少于对照组,差异有统计学意义(P<0.01);观察组腹腔镜术中对患者诊断准确率明显高于对照组(P<0.05);观察组患者腹腔镜术后住院时间明显短于对照组(P<0.01)。结论将腹腔镜应用于普外科急腹症诊断与治疗过程中,能够在很大程度上缩短急腹症患者的住院时间,减少创伤,与传统的剖腹探查手术相比较,其优势非常明显,值得推广应用。

Application research on laparoscopic diagnosis and treatment of acute abdomen in the department of general surgery

Objective To explore the diagnosis and treatment of laparoscopy in acute abdomen in department of general surgery and clinical application.Methods Retrospectively analysed the clinical data of 120 cases with acute abdomen from March 2008 to March 2012 in our hospital,the patients were randomly divided into control group and observation group,the control group(n=60)with acute abdomen laparotomy;the observation group(n=60) using a laparoscopic exploration.Operation time,hospitalization time,bleeding during the operation,the exploration results of the two groups were compared and analyzed.Results Compared the operation time,there was no statistically significant difference between the two groups(P > 0.05);in the amount of bleeding,observation o group was less than control group,the diffence was statistically significant(P < 0.01);the diagnosis accurate rate,observation group was higher than control group(P < 0.05);the hospitalization time,observation group was significantly shorter than control group(P < 0.01).Conclusion Laparoscopic diagnosis and treatment of common surgical abdomen can greatly shorten the time of hospitalization,reduce the trauma,the advantage is very clear,worthy of spreading application.
