二甲双胍联合达英-35治疗多囊卵巢综合征的临床研究

目的探讨二甲双胍联合达英-35治疗PCOS的疗效,以探讨其临床应用价值。方法采用自身对照研究和病例对照研究。纳入92例PCOS患者(其中PCOS-肥胖组和PCOS-非肥胖组各46例),测定内分泌激素及OGTT、胰岛素释放试验。结果 PCOS-肥胖组与非肥胖组比较,其对应的多毛评分、DHT、DHEA-S、血糖(60、120min)、胰岛素(30、60、120、180min)水平显著增加,有统计学意义。两种药物联用治疗前后比较,PCOS-肥胖组和非肥胖组的体重指数、多毛评分、卵巢体积有缩小,但差异无统计学意义。PCOS-肥胖组的DHT、DHEA、DHEA-S、胰岛素释放试验的各项指标均明显下降,差异有统计学意义。PCOS-非肥胖组INS0、INS30、INS60明显下降,差异有统计学意义。结论达英-35联合二甲双胍治疗能降低高胰岛素血症和高雄激素血症。

Clinical study of metformin combined with diane-35 in treating of polycystic ovary syndrome

Objective To evaluate the value of metformin combined with diane-35 in improving the polycystic ovary syndrome(PCOS) phenotype.Methods Before-after study and case-control study.ninety-two PCOS patients(46 obese and 46 non-obese) included in our study were treated with Meltormin and Diane-35 for 6 months.Endocrinal hormone,oral glucose tolerance test(OGTT) and insulin were measured before and after therapy.Results The results of DHT,DHEA-S,insulin(30,60,120,180min) were statistically significant higher in obese group than in non-obese group.BMI and hirsutism were decreased,and the size of ovary was shrinked after combination therapy in both groups,but the differences were not significant.DHT,DHEA,DHEA-S and all index of insulin were statistically significant decreased in obese group.Insulin(0,30,60min) was statistically significant decreased in non-obese group.Conclusion Metformin combined with diane-35 can improve hyperinsulinism and hyperandrogenism so would be beneficial in PCOS treatment.
