经阴道彩色多普勒超声联合阴道镜在宫颈病变诊断中的应用价值

目的探究经阴道彩色多普勒超声结合阴道镜检查在宫颈病变诊断中的应用价值。方法选取我院2010年3月～2011年3期间收治的240例经宫颈活检或手术证实的宫颈病例,将其检查结论与阴道镜、阴道彩超检查结果进行对比验证。结果宫颈炎的阴道彩超诊断符合率为74.6%,误诊率为25.4%;阴道镜结果为84.8%和15.2%;二者结合为96.0%和4%。宫颈肌瘤的彩超诊断符合率为84.2%,误诊率为15.8%;阴道镜结果为42.9%和57.1%;二者结合为88.9%和11.1%。宫颈癌前变彩超诊断符合率为44.4%,误诊率为55.6%;阴道镜结果为84.6%和15.4%;二者结合为100%和0%。宫颈癌彩超诊断符合率为37.5%,误诊率为62.5%;阴道镜结果为50%和50%;二者结合为83.3%和16.7%。其他病变(宫颈湿疣等)阴道彩超诊断符合率为33.3%,误诊率为66.7%;阴道镜结果为40%和60%;二者结合为66.7%和33.3%。结论阴道彩色多普勒超声联合阴道镜检查的方法能有效提高宫颈病症的确诊率,在宫颈病变的临床诊断过程中有较高应用价值。

The application of transvaginal color doppler integrated with colposcopy in the diagnosis of cervical lesions

Objective To explore the application of transvaginal color doppler integrated with colposcopy in the diagnosis of cervical lesions.Methods 240 confirmed cases from 2010.3～2011.3 were used to be compared with the results of colposcopy and transvaginal color doppler(TVCD).Results Cervicitis: diagnose accordance rate of TVCD is 74.6%,the misdiagnosis rate is 25.4%;Colposcopy result is 84.8% respectively and 15.2%;Integration result is respectively 96.0% and 4%.Cervical Myoma: Diagnose accordance rate of TVCD is 84.2%,the misdiagnosis rate is 15.8%;Colposcopy result is respectively 42.9% and 57.1%;Integration result is respectively 88.9% and 11.1%.CIN: Diagnose accordance rate of TVCD is 44.4%,the misdiagnosis rate is 55.6%;Colposcopy result is respectively 84.6% and 15.4%;Integration result is respectively 100% and 0.Cervical Carcinoma: diagnose accordance rate of TVCD is 37.5%,the misdiagnosis rate is 62.5%;Colposcopy result is respectively 50% and 50%;Integration result is respectively 83.3% and 16.7%.Others: Diagnose accordance rate of TVCD is 33.3%,the misdiagnosis rate is 66.7%;Colposcopy result is respectively 40% and 60%;Integration result is respectively 66.7% and 33.3%.Conclusion The application of transvaginal Color doppler integrated with colposcopy will effectively improve the diagnostic rate of cervical lesions,and has a high value in the process of its clinical diagnosis.
