高海拔地区肺叶切除术后呼吸功能变化的探讨(附35例报告)

摘    要:目的探讨在高海拔地区(海拔2260m)行肺叶切除术对患者的肺功能、血气分析的变化,生活质量、生存期的影响。方法总结35例肺叶切除患者年龄、肺功能变化、血气变化。并行统计学处理。结果①高海拔地区肺叶切除术后不同年龄组肺功能最大通气量(MVV)、肺活量(VC)的下降比平原地区明显,差别有高度的显著性(P<0.01)。术后血氧饱和度(SaO2)上升明显(P<0.01),但二氧化碳分压(PCO2)略有下降(P<0.01)。结论①高海拔地区肺叶切除术后肺功能损害比平原地区严重;②肺功能的代偿和年龄有明显的差别,年龄越大其肺功能的代偿能力越差;③术后提高了肺通气和换气功能,改善了通气/血流比,SaO2上升,PCO2略有下降;④通过上述资料探讨高海拔地区肺叶切除的相关问题。

Abstract:Objective: To explore the changes of respiratory function after pulmonary lobectomy in xining. Methods: Documents of 35 cases with pulmonary lobectomy were reviewed on age, lung function and blood gas. Results: Levels of MVV, VC after pulmonary lobectomy were decreased significantly compared with the sea level group (P<0.01). Level of SaO2 increased noticeably (P<0.01) but PCO2 decreased (P<0.01). Conclusions: There is severe functional damage of lung after pulmonary lobectomy at high altitude. And compensation of lung function has a difference among various age's groups and a bad compensation occurs with greater age; Pulmonary ventilation and exchange are increased after pulmonary lobectomy.
