体脂率对不育男性精液质量的影响及相关性研究

摘    要:目的探究体脂率(BFP)对不育男性精液质量的影响及相关性研究。方法选取2016年7月至2018年9月成都市妇女儿童中心医院诊治的128例继发性不育男性为研究对象。根据不同BFP将其分为A组(BFP<15%,n=40)、B组(15%≤BFP<20%,n=46)、C组(BFP≥20%,n=42),比较三组患者精液质量参数及体外受精结局(受精率、优质胚胎率),采用Pearson相关分析法研究BFP与精液质量参数及受精率、优质胚胎率的相关性,多因素Logistic回归分析研究受精率及优质胚胎率的影响因素。结果 B组精子前向运动率、精子浓度、精液量高于A、C组,差异具有统计学意义(P<0.05);A组精子前向运动率、精子浓度、精液量较C组无统计学差异(P>0.05);B组受精率、优质胚胎率高于A、C组,差异具有统计学意义(P<0.05);A组受精率、优质胚胎率较C组无统计学差异(P>0.05);BFP与精液量呈负相关(P<0.05),BFP与精子浓度、精子前向运动率、受精率、优质胚胎率无相关性(P>0.05);精子浓度、BFP、精液量是受精率的独立影响因素(P<0.05);精子浓度、BFP是优质胚胎率的独立影响因素(P<0.05)。结论 BFP与不育男性精液质量及体外受精结局相关,体脂率过高或过低均会对精液质量及受精率、优质胚胎率产生不良影响,由此说明控制体脂含量能够改善精子质量及受精结局,恢复男性生殖能力。

Abstract:Objective To investigate the effect of body fat percentage(BFP) on the semen quality in infertile men and its correlation. Methods 128 secondary infertile men admitted to Chengdu Women′s and Children′s Central Hospital from July 2016 to September 2018 were selected, and according to the different BFP, were divided into group A(BFP<15%, n=40), group B(15%≤BFP<20%, n=46), and group C(BFP≥20%, n=42). The semen quality parameters and in vitro fertilization outcomes(fertilization rate, high quality embryo rate) of the three groups were compared. Pearson correlation analysis was used to study the correlation between BFP and semen quality parameters, fertilization rate, and high-quality embryo rate. Then multivariate Logistic regression analysis was used to determine the influencing factors of fertilization rate and high quality embryo rate. Results The forward motility rate, sperm concentration, and semen volume were significantly higher in group B than those in group A and C(P<0.05), but no statistically significant difference was found between group A and group C(P>0.05). The fertilization rate and high quality embryo rate were significantly higher in group B than those in group A and C(P<0.05), with no difference between group A and group C(P>0.05). BFP was negatively correlated with semen volume(P<0.05), while was not correlated with sperm concentration, sperm forward motion rate, fertilization rate, and high quality embryo rate(P>0.05). Semen volume, sperm concentration and BFP were independent factors of fertilization rate(P<0.05). Sperm concentration and BFP were independent influencing factors of high quality embryos(P<0.05). Conclusions BFP is related to semen quality and in vitro fertilization outcome of infertile men. Excessive or low body fat rate will have adverse effects on semen quality, fertilization rate and high quality embryo rate, which indicates that controlling body fat content can improve sperm quality and fertilization outcome and restore male reproductive capacity.
