摘    要:目的探讨长链非编码RNA H19(LncRNA H19)和微小RNA-200a(miR-200a)在前列腺癌中的表达及其与患者临床病理参数及预后的关系。方法选取2013年1月至2015年8月武汉市武昌医院诊治的68例前列腺癌患者作为研究对象。将这68例患者设为研究组,另选取同期前列腺增生患者42例为对照组。采用实时荧光定量(qRT-PCR)法检测LncRNA H19、miR-200a表达水平,采用免疫层析法检测血清前列腺组织特异性抗原(PSA)水平。根据检测结果将前列腺癌患者分为LncRNA H19分为高表达组(40例)与低表达组(28例);miR-200a分为高表达组(30例)与低表达组(38例),观察其与患者临床病理参数的关系。采用Pearson法进行相关性分析;采用Kaplan-Meier法绘制前列腺癌患者3年的生存曲线;COX回归模型分析影响前列腺癌患者预后的相关因素。结果与对照组相比,研究组LncRNA H19表达水平显著升高,差异具有统计学意义(P<0.01),miR-200a表达水平显著降低,差异具有统计学意义(P<0.01);LncRNA H19、miR-200a表达均与淋巴结转移、病理分期、PSA表达水平及Gleason评分有关,差异具有统计学意义(P<0.05);Pearson分析显示LncRNA H19与miR-200a呈负相关,差异具有统计学意义(P<0.01);LncRNA H19高表达组与miR-200a低表达组PFS、OS均分别显著低于低表达组、高表达组,差异具有统计学意义(P<0.05);Cox分析显示,淋巴结转移、PSA表达水平、Gleason评分以及LncRNA H19与miR-200a表达水平均为影响前列腺癌患者预后的危险因素。结论 LncRNA H19在前列腺癌组织中表达上调,而miR-200a表达下调,两者呈负相关,且均与患者临床病理参数及预后有关,可作为临床诊断、治疗前列腺癌及评估预后的潜在分子靶点。

Abstract:Objective To investigate the expressions of long non-coding RNA H19(LncRNA H19)and microRNA-200 a(miR-200 a)in prostate cancer and their relationships with clinicopathological parameters and prognosis.Methods From January 2013 to August 2015,68 patients with prostate cancer diagnosed and treated in Wuhan Wuchang Hospital were selected as the study objects.The 68 patients were divided into study group,and another 42 patients with BPH in the same period were selected as control group.The expression levels of LncRNA H19 and miR-200 a were detected by real-time fluorescence quantitative(qRT-PCR),and the level of serum prostate tissue-specific antigen(PSA)was detected by immunochromatography.The prostate cancer patients were divided into LncRNA H19 high expression group(40 cases)and low expression group(28 cases)according to the test results of LncRNA H19;and they were divided into miR-200 a high expression group(30 cases)and miR-200 a low expression group(38 cases)according to the test results of miR-200 a,and the relationship between them and clinical pathological parameters was observed.Pearson method was used for correlation analysis;Kaplan-Meier method was used to draw the 3-year survival curve of prostate cancer patients;and COX regression model was used to analyze the prognostic factors of prostate cancer patients.Results Compared with the control group,the expression level of LncRNA H19 in the study group was significantly higher,and the difference was statistically significant(P<0.01),the expression level of miR-200 a was significantly lower,and the difference was statistically significant(P<0.01);the expressions of LncRNA H19 and miR-200 a were related to lymph node metastasis,pathological stage,PSA expression level and Gleason score,and the differences were statistically significant(P<0.05);Pearson analysis showed that LncRNA H19 was negatively correlated with miR-200 a,and the difference was statistically significant(P<0.01);the PFS and OS of LncRNA H19 high expression group and miR-200 a low expression group were significantly lower than those of LncRNA H19 low expression group and miR-200 a high expression group respectively,and the differences were statistically significant(P<0.05);COX analysis showed that lymph node metastasis,PSA expression level,Gleason score,and LncRNA H19 and miR-200 a expression levels were all risk factors for the prognosis of prostate cancer patients.Conclusions The expression of LncRNA H19 is up-regulated in prostate cancer,while miR-200 a is down regulated.They are negatively correlated and both of them are related to the clinicopathological parameters and prognosis of patients,which can be used as potential molecular targets for clinical diagnosis,treatment and prognosis evaluation of prostate cancer.
