60岁以上女性泌尿道感染患者合并糖尿病与否对临床治疗效果的影响

目的分析60岁以上女性泌尿道感染患者合并糖尿病与否对临床治疗效果的影响。方法选取2018年2月至2019年2月杭州市红十字会医院诊治的60岁以上女性泌尿道感染的55例患者作为研究对象。依据是否合并糖尿病将其分为合并糖尿病的泌尿道感染观察组和单纯泌尿道感染的对照组,观察组有15例患者,对照组有40例患者。两组患者入院后均实施哌拉西林+头孢克肟治疗,持续治疗14d后,对两组患者临床治疗效果进行比较。结果治疗前,两组患者降钙素原、中性粒细胞百分比、C-反应蛋白、症状积分(尿频、尿急、尿痛)比较,其差异均无统计学意义(均P>0.05);治疗后7d和治疗后14d,两组患者降钙素原、中性粒细胞百分比、C-反应蛋白、症状积分均比治疗前明显下降,组间、不同时间点相比较,其差异均具有统计学意义(均P<0.05);随访3个月,观察组患者复发率高于对照组患者,其差异具有统计学意义(P<0.05)。结论是否合并糖尿病对60岁以上女性泌尿道感染患者临床效果具有直接影响,合并糖尿病患者疗效较差,复发风险更高。

Influence of urinary tract infection combined with diabetes mellitus or not on the clinical therapeutic effect in women over 60 years old

Objective To analyze the effect of diabetes in women with urinary tract infection(UTI) over 60 years of age on clinical treatment. Methods 55 patients with urinary tract infections over 60 years old who were diagnosed and treated in Hangzhou Red Cross Hospital from February 2018 to February 2019 were selected as the research subjects, and they were divided into the urinary tract infection observation group with diabetes and the control group of urinary tract infections alone, there were 15 patients in the observation group and 40 patients in the control group. Both groups of patients were treated with piperacillin + cefixime after admission. After 14 days of continuous indicators, the clinical treatment effects of the two groups were compared. Results Before treatment, there was no significant difference in the procalcitonin, neutrophil percentage, C-reactive protein and symptom score(frequency of urination, urgency of urination, pain of urination) between the two groups(P>0.05). The procalcitonin, neutrophil percentage, C-reactive protein and symptom score of the two groups on the 7 th and the 14 th day after treatment were significantly lower than those before treatment, and there were significant differences between the groups and different time points(all P<0.05). After 3 months of follow-up, the recurrence rate in the observation group was higher than that in the control group, and the difference was statistically significant(P<0.05). Conclusions Whether diabetes mellitus has a direct impact on the clinical effect of urinary tract infection in women over 60 years old, and the patients with diabetes mellitus have poor curative effect and higher risk of recurrence.
