临床路径在社区高血压患者中的应用

目的探讨临床路径在社区高血压患者中的应用效果。方法将535例上海市罗店社区的高血压患者按随机数字表法分为2组：干预组246例和对照组289例。干预组采用临床路径进行干预，对照组采用社区防治模式进行干预。同时对2组患者采用降压药物治疗。观察2组患者干预后满意度、药物治疗依从性及血压『收缩压（SBP）、舒张压（DBP）]控制水平等情况。结果干预组患者干预后满意度显著高于对照组，差异有统计学意义（P〈0．05）；干预组患者干预后药物治疗依从性显著高于对照组，差异有统计学意义（P〈0．05）；干预组患者干预后SBP、DBP控制水平均优于对照组（均P〈0．05）。结论实施临床路径提高了患者的自我管理能力，增加了患者的高血压控制效果，是提高社区高血压患者疗效的可行模式。

Application of Clinical Pathways in Community Patients with Hypertension

Objective To explore the efficacy of clinical hypertension. Methods A total of 535 patients with hypertension pathways community patients in Luodian community of Shanghai with city were randomly divided into two groups. The intervention group （n=246）was treated on clinical pathways and control group （n=289） was given community prevention and treatment. At the same time, all patients received antihypertensive drug treatment. Patient satisfaction,medication compliance and levels of systolic blood pressure（SBP）and diastolic blood pressure（DBP）control were observed in both groups. Results Compared with control group, patient satisfaction, medication compliance and increased in intervention group （all P〈0.05）. Conclusion The levels of blood pressure control significantly implementation of clinical pathways enhances self-management skills and improves the effect of blood pressure control in patients with hypertension. Therefore, clinical pathways are feasible for increasing the efficacy in community patients with hypertension.
