卡介菌多糖核酸辅助治疗初治涂阳肺结核的疗效

摘    要:目的探讨卡介菌多糖核酸(BCG-PSN)辅助治疗初治涂阳肺结核的疗效。方法将117例初治涂阳肺结核患者按确诊并建立病案的顺序随机分成治疗组(60例)和对照组(57例)。并按《中国结核病防治规划工作指南》要求,对患者实行全程督导化疗与考核。对照组采用单纯化疗(化疗方案为2H3R3Z3E3)治疗;治疗组在对照组化疗的基础上加用BCG-PSN 1mL肌内注射,2次.周-1。2组均8周为1个疗程。分别在治疗前、治疗后2个月末进行痰检、胸片检查,对2组治疗后2个月末痰涂片阴转及病灶吸收情况进行比较。结果治疗组和对照组治疗后2个月末的痰菌阴转率分别为95.0%和78.9%,病灶吸收率分别为83.3%和57.9%,2组比较差异均有统计学意义(P〈0.05或P〈0.01)。结论 BCG-PSN辅助治疗初治涂阳肺结核患者效果明显,能加快结核病变的吸收及痰菌的阴转速度。

Abstract:Objective To investigate the effect of adjuvant BCG-polysaccharide nucleic acid on new smear-positive pulmonary tuberculosis(TB).MethodsA total of 117 smear-positive tuberculosis patients were randomly divided into treatment group(60 patients) and control group(57 patients) according to the diagnosis and medical records.Patients underwent full supervision and the efficacy was assessed according to "china TB control planning guide".All patients were given routine chemotherapy scheme 2H3R3Z3E3 within 2 months.Simultaneously,the treatment group was intramuscularly administered BCG-polysaccharide nucleic acid(1 mL) twice per week.The chest x-ray and sputum examination were performed before and at the end of 8 weeks of treatment.The sputum negative conversion rate and absorption rate of infection focus were compared between the two groups.ResultsCompared with control group,both sputum negative conversion rate and lesion absorption rate were significantly increased in treatment group(95.0% and 83.3% vs 78.9% and 57.9%,respectively;P0.05 or P0.01).ConclusionThe adjuvant therapy with BCG-polysaccharide nucleic acid can effectively promote lesion resolution and sputum negative conversion in new smear-positive pulmonary TB patients.
