腰椎管单开门成形术治疗腰椎管狭窄症40例

作者报告采用腰椎管单开门成形术治疗腰椎管狭窄症．1993年4月～1996年4月手术40例，平均49岁，病史2～5年，平均3．2年。40例经长期系统保守治疗正疗程以上．均经X线平片、CT和／或脊髓造影检查。术中见骨性及发育性30例，肿瘤性4例，炎症性2例．其他4例。术后卧床1月。随访3月～36月，优26例，良12例，可2例。该术式可扩大骨性椎管容积；同时切除增生黄韧带、小关节突及突出的髓核．直接解除压迫；可探查同侧各神经根管；能保持后柱结构大部完整，操作简单。可作为一种探查减压术式。

Unilateral Opening Door Laminoplasty for Lumbar Spinal Stenosis-a Report of 40 Cases

The authers reported that 40 patients with lumbar spinal stenosis were treated with unilateral opening door laminoplasty from 1993 t0 1996. The mean age was 49. All patients had suffered from the disease for 2 to 5 years (mean 3. 2y) and had been treated conservatively for more than one course. The diagnosis was confirmed by X-rayfilm, CT and/or myolography and was classified as bony and devclopemental stenosis (30 cases), tumoral stenosis (4 cases). inflammatory stenosis(2 cases) and others (4 cases) after operation. All patients were kept in bed for one month and followed up for 3 to 36 months. The Tesults were excellent in 26 cases, good in 12 and fair in 2. The advantages of this method are as follows: 1. bony vertebral canal enlarged; 2. the thick ligamentum flavum, degenerative articular process and the prolapsed dise excised. and therefore, the compression relicved directly; 3. all nerve root canals of the same side inspected;4. the construction of the popsterior spinal column nearly intact; 5. The operation not difficult. So it can be used as an altemative methed to inspect and decompress lumbar spinal canal.
