导管相关性血流感染患者原因分析与预防控制

目的 探讨导管相关性血流感染的原因及对策.方法 选择2007年3月-2010年3月共450例多途径中心静脉置管病例,其中,发生导管相关性血流感染者10例.10例分别从留置导管中和其他外周血管采血并且剪下导管尖端培养,均培养出同种细菌,1例培养出白色念珠菌.结果 本组病例从发现后24 h内即明确诊断导管相关性血流感染的患者8例（占80%）,就诊后2-4 d内明确诊断者2例（占20%）.其中内科感染例数为7例,感染率为1 75%;外科感染例数为2例,感染率为6 67%;其他感染例数为1例,感染率为5%.结论 应提前预防控制导管相关性血流感染,从而缩短住院时间,节约医疗资源,减轻患者痛苦.

Catheter - related bloodstream infection cause analysis and prevention and control

Objective To explore and discuss the cause of the catheter - related bloodstream infection and its countermeasures. Methods Select 450 cases of multi - channel central venous catheter that were treated from March 2007 till March 2010, among which ten cases were those of catheter - related bloodstream infection. For the 10 cases, blood was respectively collected from the original catheter as well as from the other peripheral catheter, and cut the catheter tip for cultivation. As a result, the same species of bacteria were cultivated, with one case resulting in the white candida albicans cultivated. Resuits 8 cases were diagnosed as catheter - related blood stream infection within 24 hours （ covering 80% ） and two patients were definitely treated within 2 -4 days after the diagnosis （ covering 20% ）. Among these ten patients,seven were cases of infection,the infection rate being 1.75% ;two patients were cases of surgery infection,the infection rate being 6. 67% ;and only one patient was diagnosed as other kind of infection, the infection rate being 5%. Conclusion Prevention and control should be made ahead of catheter - related bloodstream infection, in order to shorten the length of hospital stay, to save health care resources and to reduce patients suffering.
