立体定向引导显微手术切除颅内较小占位性病变

摘    要:目的探讨立体定向技术切除颅内较小占位病变的方法及引导显微手术切除的优越性。方法采用瑞典Leksell-G型立体定向仪及手术计划系统对42例患者49个颅内较小占位病变行术前精确定位后,环钻开颅,在显微镜下切除病灶。结果所有病例均为全切,病变性质脑膜瘤9例(其中2个部位2例),转移瘤8例(其中2个部位2例,3个部位1例),胶质瘤7例,脑脓肿7例,脑囊虫5例,3例结核瘤,2例海绵状血管瘤,1例血管母细胞瘤,无手术并发症及死亡病例。结论立体定向引导显微手术切除颅内较小占位病变是一种定位精确,创伤轻微,安全和有效的手术方法,特别在颅内多发及功能区占位病变切除方面有无可替代的优越性。

Abstract:Objective To study the methods of microsurgical excision of small intracranial lesions by stereotactic guiding.Methods 49 small intracranial lesions of 42 patients were excised by microsurgery after localized by Leksell-G stereotaxy and surgical planning system.Results Total tumor removal was achieved in all cases.Pathology:9 meningiomas (2 cases with 2 lesions),8 metastatic tumors (2 cases with 2 lesions,1 case with 3 lesions),7 gliomas,7 brain abscess,5 cerebral cysticercoses,3 brain tuberculomas,2 angiocavernoma,1 hemangioblastoma.No surgical complications and death case happened.Conclusion Microsurgical excision of small intracranial lesions by stereotactic guiding is an accurate location,minimal injury,safe and effective technique,especially for multiple lesions in functional area.
