摘    要:目的观察急性心肌梗死(AMI)后早期运动训练对左心室重构、泵功能及心肌细胞凋亡的影响及可能机制。方法 30只SD大鼠随机分为假手术组(Sham组)、心梗对照组(AMI-Sed组)和心梗+运动训练组(AMI-Ex组)。AMI模型制作成功后,AMI-Ex组进行8周跑台训练。AMI后9周,进行心脏组织病理学观察,测量心脏指数及Caspase-3和Caspase-9活性,左心室导管法测量血流动力学参数,流式细胞法检测细胞凋亡指数,Western-blotting法检测细胞色素C的释放。结果 AMI-Ex组与AMI-Sed组比较,右室心脏质量指数、肺含水率、左心室压力负荷最大值、心肌细胞凋亡率、Caspase-3和Caspase-9活化水平及细胞色素C释放显著降低。左心室压峰值和左心室压力变化速度正负最大值显著升高。结论 AMI后早期运动训练可以抑制左心室重构以及泵功能障碍,其机制可能与抑制线粒体凋亡通路从而降低非梗死区心肌细胞凋亡率有关。
