桥本甲状腺肿合并非霍奇金淋巴瘤4例临床病理分析

目的探讨桥本甲状腺肿合并非霍奇金淋巴瘤(NHL)的临床病理特点。方法通过HE及免疫组化染色观察4例桥本甲状腺肿合并NHL的形态特征,并复习文献。结果4例桥本甲状腺肿,3例合并黏膜相关淋巴样组织结外边缘区B细胞淋巴瘤(MALToma);1例合并弥漫性大B细胞淋巴瘤(DLBCL),结合细针穿刺细胞学检查(FNAC)、术中冰冻病理检查可形成桥本淋巴瘤的提示性诊断,最后经石蜡切片及免疫组化确诊并明确组织学类型。结论提高对桥本甲状腺肿合并NHL临床病理特点的认识,可避免桥本甲状腺肿合并NHL的漏诊和误诊。

The clinicopathological analysis of Hashimoto thyroiditis complicated with thyroid non-Hodgkin's lymphoma

Objective To explore the clinicopathological characteristics of Hashimoto thyroiditis complicated with thyroid non-Hodgkin's lymphoma(NHL).Methods Four cases of Hashimoto thyroiditis complicated with NHL were observed with HE and immunohistochemical stains with literature review.Results Four cases of Hashimoto thyroiditis complicated with NHL were enrolled.Three cases of Hashimoto thyroiditis were complicated with mucous associated lymphoid tissue malignant lymphoma(MALToma).One was with diffuse large B-cell lymphoma(DLBCL).Pathological frozen section during operation together with fine-needle aspiration cytology(FNAC)could make a suggestive diagnosis of Hashimoto thyroiditis complicated with NHL,which could be diagnosed finally with pathological paraffin section and immunohistochemistry.Then the histological type was determined.Conclusion It is necessary to strengthen the recognition of the clinicopathological characteristics of Hashimoto thyroiditis complicated with thyroid NHL,which may avoid missed and mistaken diagnosis.
