肺结核患者外周血记忆性T细胞及B细胞亚群的检测分析

摘    要:目的探讨肺结核患者外周血记忆性T及B细胞亚群的表达水平。方法采用流式细胞仪检测肺结核患者(TB)、潜伏感染者(LTBI)、健康人(HD)外周血记忆性T、B细胞亚群水平及B细胞表达水平;利用Elispot技术检测外周血单个核细胞(PBMC)结核菌抗原特异性IFN-r分泌水平。结果流式结果显示,TB及LTB1的CD4*及CD8*中心记忆性T细胞(Tcm)显著增多,以CD4*尤为显著;相反,效应记忆性T细胞(Tem)减少。TB记忆性B细胞均低于HD和LTBI(P〈0.01);而在HD与LTBI及TB这三组CD19&细胞占淋巴细胞比例中都有明显差异(P〈0.01;P〈0.001)。CD4’/CD8’Tcm与IFN-γ ELISPOT结果成正相关。结论肺结核患者外周血中存在着记忆性T及B细胞亚群的紊乱,中心记忆性T细胞增多,效应记忆性T细胞减少,记忆性T、B细胞的数量及免疫状态与结核菌感染的不同转归有关,记忆性T、B细胞在结核病发病机制中发挥着重要作用。

Abstract:Objective To study the expression of memory T cell and B cell subsets of peripheral blood in patients with pulmonary tuberculosis. Methods The memory T cell and B cell subsets of peripheral blood were analyzing with flow cytomety ; Antigen specific-IFN-γ response were determined in PBMCs by using in-house Elispot. Results CD4+ and CD8+ central memory T cells in TB and LTBI were much higher ,especially CD4+ . On the contrary, effective memory T cells were reduced. Memory B ceils in TB were decreased compared with HD and LTBI. ( P 〈0. 01 ,P 〈0. 001 ). The proportion of CD4 ,CD8+ central memory T ceils is signi? cantly correlated with the released levels of specific IFN-γ. Conclusion Memory T and B cell subsets were confused in peripheral blood of patients with pulmonary tuberculosis, Central memory T cell increased, while effort memory T cell decreased ; the population of memory T/B cells and immu- nization were associated with the result of Mycobacterium tuberculosis infection. Memory T and B cells play an important role in the tuberculosis mechanism.
