急诊胸痹患者的全因死亡GRACE预测积分分析

目的观察急诊胸痹患者的全因死亡GRACE预测积分情况，提高急诊胸痹患者的危险度辨识水平。方法对67例胸痹患者进行辨证，根据不同证型分为气滞血瘀组、痰浊闭塞组、阴血虚证组和阳气虚证组4组．登记4组患者的全因死亡GRACE预测积分中的相关项目，并对数据进行统计分析。结果气滞血瘀组全因死亡GRACE预测积分为（129．17±42．32）分，痰浊闭塞组为（124．41±32．24）分，阴血虚证组为（130．43±43．17）分和阳气虚证组为（180．00±26．83）分，差异有统计学意义（P〈0．01）；阳气虚证组的高危组构成比较有统计学意义（P〈0．01）；阳气虚证组的心电图改变的差异有统计学意义（P〈0．01）。结论全因死亡GRACE预测积分可增加胸痹患者中医辨证分型诊断的科学性。阳气虚证的胸痹患者的GRACE积分较高，预示着其死亡率也较高，值得重视。

Analyse of GRACE Risk Score about Chest Stuffiness Patients of Different Syndromes in Emergency

Objective： To research the GRACE risk score about chest stuffiness patients of different syndromes in emergency and improving the level of identification about the criticality of chest stuffiness patients. Methods： 67 chest stuffiness patients in emergency were divided into 4 groups according to syndrome differentiation ir~clud- ing the group of qi stagnation blood stasis,the group of stagnation of phlegm-dampness,the group of deficiency of yin blood and the group of deficiency of yang qi. It was counted and researched the data of the case of patients about GRACE risk score. Results： GRACE risk score of the group of qi stagnation blood stasis was （129.17± 42.32） ,the group of stagnation of phlegm-dampness was （124.41±32.24） ,the group of deficiency of yin blood was （130.43±43.17） and the group of deficiency of yang qi was （180.00±26.83）. The difference of proportion of the high risk group in deficiency of yang qi was significant（P〈 0.01 ）. The change of ECG in deficiency of yang qi was significant （P〈 0.01 ）. Conclusion： GRACE risk score can improve the level of diagnose identification about syndrome differentiation in chest stuffiness patients. GRACE risk score of the group of deficiency of Yangqi is so high that it should be pay attention.
