颅内血肿穿刺碎吸清除术治疗急性重型脑出血的临床研究

摘    要:目的:探讨应用颅内血肿穿刺碎吸清除术治疗急性重型脑出血在降低病死率,致残率、提高治愈率及患者生存质量方面的作用及安全性。方法:将1993年1月~2002年12月应用颅内血肿穿刺碎吸清除术治疗的急性重型脑出血32例作为治疗组,随机抽取同一时期我科锥颅抽取血肿治疗的急性重型脑出血33例作为对照组。观察两组患者的血肿清除(吸收)情况,治疗前后神经功能恢复情况及治疗的有效率、病死率、病残率。结果:治疗组血肿清除率、神经功能恢复情况、治疗的有效率、病死率、致残率与对照组比较均有显著差异(P<0.01)。结论:应用颅内血肿穿刺碎吸清除术治疗急性重型脑出血能有效地降低病死率、致残率,促进神经功能恢复,提高治愈率及患者的生存质量,治疗方法安全可靠。

Abstract:Objective:To discuss the Safety of a micro-invasive surgery of hematoma evacuation technique for acute severe cerebral hemorrhage and the effect on reducing death and disability rates,enhancing heal rate and improving life quality.Methods:32 patients with acute severe cerebral hemorrhage who received micro-invasive surgery of hematoma evacuation as treatment group.Choosing same time another 33 patients with acute severe cerebral hemorrhage who received craniotomy to remove the blood as control group.Absorption of hematoma,functional recovery of the nerve,effective rate,mortality and disability rate were analyzed.Results:Compare with control group,there are a higher hematoma absorption rate,effective rate and a lower mortality,better functional recovery of the nerve in treatment group(P<0.01).Conclusion:for acute severe cerebral hemorrhage,the microinvasive surgery of hematoma evacuation technique is safe and reliable,it can reduce the death and disability rate,improve functional recovery of the nerve and patient's life quality.
