冠心病合并糖尿病患者应用国产双降解药物洗脱支架疗效观察

目的 评价国产双降解Excel药物洗脱支架(DES)在冠心病合并糖尿病(DM)患者介入治疗中的临床疗效及安全性.方法 连续入选的212例冠心病患者行经皮冠状动脉介入治疗(PCI),其中DM者78例,非DM134例,靶血管均置入ExcelDES,对比观察手术成功率、术中并发症及1～14个月随访期间的心绞痛、猝死、主要不良心脏事件(MACE)发生率及复查冠脉造影情况.结果 DM组及非DM组手术即刻成功率分别为98.7%、99.3%,320处靶病变共置入支架420枚,其中DM组病变98处,支架数126枚,直接支架29枚(23.1%),非DM组病变222处,置入支架294枚,直接支架96枚(32.7%).术中无严重并发症发生,1例非DM组心梗患者术后6h猝死.DM组临床随访74例(97.4%),造影复查18例(24.3%),非DM组分别为132例(97.1%)、24例(18.2%).DM组2例心绞痛复发,非DM组3例发生心绞痛,两组各有1例造影证实为支架内再狭窄,并行血运重建,MACE发生率分别为1.4%、0.8%.两组均无迟发性血栓发生.结论 国产涂层可生物降解Excel DES应用于冠心病合并DM患者,介入治疗安全、有效.
