妊娠输尿管结石合并肾绞痛处理原则的探讨

目的探讨妊娠期肾输尿管结石合并肾绞痛处理原则及最为简便、安全的治疗方法。方法采用回顾性分析临床试验方法，应用“留置佳”输尿管支架管对58例妊娠期肾输尿管结石合并肾绞痛的患者，在局麻下行患侧输尿管镜逆行置管术。比较置管前后患者相关症状、体征变化，肾绞痛、肾积水缓解的程度，尿常规检查，彩超检查等。结果58例患者在输尿管镜下患侧逆行置入“留置佳”双J管后，肾绞痛症状均缓解消失，尿常规检查尿中红细胞、白细胞计数减少，彩超检查肾积水减轻或消失。全组患者足月分娩后再行处理结石并拔除输尿管支架管，无流产病例出现。结论应用“留置佳”输尿管支架管治疗妊娠期肾输尿管结石合并顽固性肾绞痛，是简便、安全、有效的治疗方法，可避免药物对胎儿的影响及手术干预带来的风险。

Discussion of treatment principles to ureteral calculi merger of renal colic during pregnancy

Objective To explore kidney ureteral calculi combine renal colic treatment principles and the most simple and safe treatment method during pregnancy. Methods Adopting the method of retrospective analysis clinical trials. 58 cases kidney ureteral calculi with renal colic in pregnancy were inserted Inlay Optima double-J-catheter with ureteroseopy under topical anesthesia. Before and after treatment ,Analyzed related symptoms ,physical signs, changes of the pregnant women, The degree of renal colic and hydronephrosis relieve,urine routine,color doppler examination. Results After lnlay optima insertion ,58 cases renal colic symptoms were disappeared. RBC counts and WBC counts in the urine were significantly de- creased. Color doppler examination hydronephrosis were reduced or disappeared. All the pregnate women were delivered healthy babies. And then dealt with calculi,pull out double-J-catheter. Conclusion For pregnant women with refractory renal colic due to kidney ureteric calculi ,retrograte Inlay Optima double-J-catheter are simple ,safe and effective therapy. Can avoid the influence of the drug on the fetus and the risks of surgical intervention.
