重症患者血脂水平与心脑血管疾病的相关性

摘    要:目的探讨血脂水平与冠心病、脑卒中的相关性。方法选择301例重症医学科住院患者,分为非冠心病+非脑卒中组(n=158)、单纯冠心病组(n=62)、单纯脑卒中组(n=57)、冠心病+脑卒中组(n=24),分析4组间血清低密度脂蛋白(low density lipoprotein,LDL)、高密度脂蛋白(high density lipoprotein,HDL)、总胆固醇(total cholesterol,TC)、甘油三脂(triglycerides,TG)共4项指标入院值、最大值差异。结果①非冠心病+非脑卒中组和单纯脑卒中组入院时TC、最大TC、最大HDL、入院时LDL、最大LDL差异具有统计学意义(P〈0.05)。②非冠心病+非脑卒中组和单纯冠心病组TG最大、最大HDL、人院时LDL、最大LDL差异具有统计学意义(P〈0.05)。③非冠心病+非脑卒中组和冠心病+脑卒中组人院时TC、最大TC、TG入院、入院时LDL、最大LDL、最大HDL异常、入院时TC异常、最大TC异常、最大LDI,异常方面差异具有统计学意义(P〈0.05)。④单纯冠心病组和冠心病+脑卒中组在最大TC异常、TG最大异常方面差异具有统计学意义(P〈0.05)。结论LDL、HDL与冠心病、脑卒中有关,TG与冠心病有关,TC与脑卒中有关,在存在冠心病的基础上TG与脑卒中有关。

Abstract:Objective To investigate the correlation between the changes in blood lipid levels and the coronary heart disease (CHD) and stroke. Methods A total of 301 cases hospitalizing in Intensive Care Unit (ICU) were divided into non-stroke + non-CHD group, pure stroke group, pure CHD group and CHD + stroke group. The differences of serum total cholesterol (TC) , triglycerides (TG) , high density lipoprotein (HDL), low density lipoprotein (LDL) among the four groups were anlaysed. Results ① There were significant differences (P〈0.05) between the non-CHD + non-stroke group and the stroke group in TC at admission, TC maximum, HDL maximum, LDL at admission and LDL maximum. ② There were significant differences (P〈0.05) between the non-CHD + non-stroke group and pure CHD group in TG maximum, HDL maximum, LDL at admission and LDL maximum. ③ There were significant differences (P〈0.05) between the non-CHD + non-stroke group and CHD + stroke group in TC at admission, TC maximum, TG at admission, LDL at admission, LDL maximum, abnormal HDL maximum, abnormal TC at admission, abnormal TC maximum and abnormal LDL maximum. ④ There were significant differences (P〈0. 05) between pure CHD group and CHD + stroke group in abnormal TC maximum and abnormal TG maximum. Conclusion LDL and HDL are associated with CHD and stroke. TG is associated with CHD. TC is associated with stroke. TG associated with stroke complicated with CHD.
