PRK治疗近视对角膜内皮细胞密度的影响

摘    要:为了解准分子激光角膜切削术(PRK)治疗近视对角膜内皮细胞密度的远期影响,我们对71例124眼分别于术前、术后3个月及术后4年用非接触内皮显微镜进行了角膜内皮细胞密度检查。结果术前平均角膜内皮细胞密度为3098+/-283个/mm2,术后3个月平均角膜内皮细胞密度为3071±268个/mm2,术后4年平均为3010+/-294个/mm2。与生理性角膜内皮细胞在成年人年平均下降率为0.52%〔1〕比较,均无显著性差异(P>0.05)。结论:内皮细胞的丧失主要发生在术后早期。此外提示PRK对角膜内皮细胞密度亦有远期的影响。鉴于该手术的对象以青壮年为为主,在今后漫长的一生中有可能需接受内眼手术。故长期观察PRK对角膜内皮细胞密度的影响仍属必要

Abstract:Purpose:To study the long term effect of 193 nm excimer laser photorefrective keratotomy(PRK)on the human corneal endothelial cell density(ECD)for myopic treatment.Method:One hundred and twenty four eyes from seventy one patients underwent photorefrective keratotomy for myopia or myopic astigmatism.Endothelial cell density was examined before operation,and on three months,forty eight months after operation,using a noncontact specular microscope.Results:The average ECDs were 3098 /-cells/mm 2 preoperatively,3073 /-274 cells/mm 2 on three months,and 3010 /-297 cells/mm 2 on 48 months postoperatively.In comparision with physiological cell loss 0 52% yearly,there were no statistically significant differentiation(P>0 05).Conclusion:The results suggest that most cell loss took place in the first period after PRK.It is necessary to investigate the endothelial cell loss on long term effect of PRK,because the most patients undergoing PRK are young,maybe they will undergo intraocular operation in their future.
